Description

The Federal Bureau of Prisons, Field Acquisition Office in Grand Prairie, Texas intends to make a single award for the provision of an Optometrist for the Federal Correctional Institution/Federal Prison Camp in Oxford, Wisconsin. Services will be performed on individuals who are under the custody and care of the Federal Bureau of Prisons. The contractor shall be responsible for, but not limited to routine Optometry services. The individual/organization must be a graduate of an accredited School of Optometry with a current license to practice in the State of Wisconsin. The contract shall consist of a 12-month base period and four 12-month option periods for renewal. The estimated quantities for the base period and each option period is twelve sessions per month. A session equals one hour. The solicitation will be made available on or about September 17, 2004 and will be distibuted solely through the General Services Administrations Federal Business Opportunities website at www.fedbizopps.gov. Hard copies of the solicitation will not be available. The site provides downloading instructions. All future information regarding this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The anticipated date for receipt of quotes is October 4, 2004. Faith-Based and Community-Based Organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible. All responsible sources may submit a quote which will be considered.
