- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FA4600-04-T-0168 is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24. All applicable provision and clauses are included in the actual solicitation. This electronic announcement constitutes the only solicitation being issued. This procurement is set aside 100% for small business. The North American Industry Classification System (NAICS) code is 337127, and the small business size standard is 500 people. The Government intends to issue a firm fixed price purchase contract for the following: 1. Refinishing of pews/front panels/kneelers, chairs, podiums, wall paneling, stairway/balcony paneling, doors, and one window sill in SAC Memorial Chapel; 2. Repair loose holders on back of pews as needed in SAC Memorial Chapel; 3. Paint windows and add for height in SAC Memorial Chapel; 4. Recover seat cushions, kneelers, and chairs using existing foam and padding in SAC Memorial Chapel IAW the Statement of Work. 1. STATEMENT OF WORK: The contractor will need to accomplish the following items in Bldg 463, SAC Memorial Chapel: a. Mask off, clean wood and re-spray as follows: (1) 34 pews including backs and kneelers. (2) 2 front panels on pews (3) 21 chairs (4) 4 arm chairs (5) Large podium (6) Small podium (7) Paneling in front between pews and altar and on back wall (8) Paneling going up stairs and in balcony (9) 5 doors (10) 1 windows sill in balcony b. Repair loose holders on back of pews as needed c. Paint windows as follows: inside areas that are painted now (1) 4 double windows (2) 4 French windows (3) Add for height d. Recover seat cushions using existing foam. The color of the cushions shall match the current color. e. Recover kneelers using existing padding. The color of the cushions shall match the current color. f. Recover 4 armchairs and 21 other chairs using existing foam and padding. The color of the cushions shall match the current color. CHAPEL SECTION RESPONSIBILITIES: a. The chapel will provide: (1) Scheduled access to all Air Force chapel facilities at Offutt AFB. CONTRACTOR RESPONSIBILITIES: a. The Contractor will provide: (1) Personal transportation (2) All equipment and supplies necessary to accomplish items listed in SOW 2. REPORTING INSRUCTIONS Before beginning and upon completion of the contract contractor shall call the Bldg 463, SAC Memorial Chapel and arrange for a suitable date and time to begin and end work on the chapel. SAC Memorial Chapel can be reached at (402) 294-6244. The contractor shall hold the government harmless for delays caused by security. The two normal ways to access the facility will be to arrive at the entrance and call 402-294-9032/9430 for an escort or to apply for a contractor?s badge through the 55th Security Forces squadron (recommended if accessing more than once per month). 3. SAFETY The contractor and contractor representatives shall comply with Air Force safety standards at all times while on government property. In accordance with AFOSH Standards 91-8/91-501, and Federal Occupational, Health and Safety agency (OSHA) standards, safe-working practices shall be followed. These include, but are not limited to: 3.1 Wearing of seatbelts/shoulder harnesses at all times while driving on Offutt AFB. 3.2. Observe maximum speed limits, 25 MPH unless otherwise posted, on Offutt AFB. 3.3. Contractor personnel shall remove watches, rings, and jewelry prior to servicing electrical circuits. 3.4. If a unit is left in a non-operational state, it shall be tagged DO NOT OPERATE. The tag shall also include the name of the contracting company and the representative placing the tag on the equipment. The contractor shall comply with the Biomedical Equipment Maintenance Department?s current lock out I tag out policy located in their shop. 3.5. Comply with the no smoking policy within the 55 WG/HC. 3.6. Report to the SAC Memorial Chapel Maintenance Department any problems with building utilities such as blown circuit breakers and defective/broken electrical receptacles. 3.7 Comply with all 55th Wing Fire Regulations.
