- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation RFQ 552-72-04 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. This is under the North American Industrial Classification System (NAICS) code 325998 and Small Business Size Standard of 500 employees. The procurement is for providing rock salt/de-icing agent for the roadways at the Department of Veterans Affairs Medical Center, Dayton, Ohio for the period October 1, 2004, through September 30, 2005. Quotations shall include price per ton. Tonnage needed is estimated at 500. Quotations shall be submitted F.O.B. destination. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition. Commercial Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items, with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The following FAR addendums are incorporated into FAR 52.212-4 as follows: 52.217-7 (which provides an Option for Increased Quantity); 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30 of any fiscal year, unless written authorization is received from the Contracting Officer). The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-13, 52.225-16, 52.232-34.
