- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Paper copies of this solicitation will not be available. This is a commercial solicitation using Simplified Acquisition Procedures found in FAR Part 13, soliciatation no. FA4690-04-Q-R002 and is issued as a request for quotes (RFQ) on 14 September 2004. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-24. FAR clause descriptions are available via the Internet at http://farsite.hill.af.mil/. The set-aside for this solicitation is 100% small business. NAICS code: 611519. Size Standard: $6,000,000.00. This solicitation is for the a planned concessionaire contract. The contractor shall perform the work described in the statement of work (see second paragraph). Services will be performed from 1 October 2004 through 30 September 2005 with FOB: Destination. The Government will award a concessionaire contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price factors considered. The following factors shall be used to evaluate offers: price. Collect calls will not be accepted, e-mail: kelly.beck@ellsworth.af.mil. STATEMENT OF WORK FOR MASSAGE THERAPIST 1. CONTRACTOR 1.1 The contractor will ensure all therapists performing massages must be certified in massage therapy from an accredited program and be a member of a professional massage organization such as Associated Body and Massage Professional (ABMP). Contractor is responsible for all certifications 1.2 The contractor will ensure all massage therapists possess current Cardio Pulmonary Resuscitation certification. Contractor is responsible for their training. 1.3 The contractor must provide the Fitness Center Director with copies of all of the therapist?s certification prior to the therapist performing any service of their contract. Copies will be maintained at the Bellamy Fitness Center. 1.4 Massage therapists must wear neat and presentable attire while performing service. 1.5 The contractor is responsible for hiring a sufficient amount of therapists to accommodate customer demand. 1.6 The contractor is responsible for payment to all massage therapists 2. SERVICE and EQUIPMENT 2.1 The contractor will equip the massage room at their expense, to include at a minimum a worktable, towels, sheets, and any other items needed to perform the service. 2.2 The contractor will ensure each therapist maintains a minimum of 1 million dollars of personal liability insurance. The government can?t be held liable in case of an injury. 2.3 The contractor will schedule appointments and operating hours conducive with customer needs and the hours of the Bellamy Fitness Center. 3. GOVERNMENT 3.1 The Fitness center Director or representative will monitor the contractor performance. 3.2 The Fitness Center Director will provide advertisement and marketing for the massage service. 3.3 The contractor will be notified one month prior to scheduled holidays, and projected down days, or any days the Fitness Center may be closed. However, down day notification can?t be made until the base commander notifies the base and it may be on a short notice basis of 1 day prior to the down day. 4. LOCATION 4.1 A private room will be provided in the Bellamy Fitness Center, Bldg 7709, Ellsworth AFB, SD 57706 5. PAYMENT 5.1 The 28 Services Squadron /Fitness Center will retain 20% of all massage service monthly earnings. 5.2 The contractor will ensure all payments for service are made at the Bellamy Fitness Center front counter. 5.3 The contractor will submit an invoice to a designated Fitness Center representative, by the 3rd calendar day of the month. 5.4 Payment to the contractor will be made by direct deposit only. 5.5 Payment distribution is as follows, prices are subject to change government 20% contractor 80% 90 minute massage $70.00 $14.00 $56.00 60 minute massage $50.00 $10.00 $40.00 30 minute massage $25.00 $5.00 $20.00 15 minute chair massage $15.00 $3.00 $12.00 Special Packages Five 60 min massages $200.00 $40.00 $160.00 Five 30 min massages $100.00 $20.00 $80.00 Five 15 min massages $60.00 $12.00 $48.00 6.
