REQUEST FOR QUOTE ? RADIOLOGY/MEDICAL TRANSCRIPTION SERVICES (NON-PERSONAL SERVICES) THIS IS A ?PLANNED? PURCHASE PENDING AVAILABILITY OF FUNDS FOR FISCAL YEAR 2005. The Medical Facility at Little Rock Air Force Base plans to award, pending funding for FY05, a Firm Fixed Price service and maintenance contract for the monthly inspection and repair when required of two Dover passenger elevators. The period of performance will be a one-year base (1 October 2004 through 30 September 2005). The Contractor shall provide all management, superivsiion, labor, equipment, tools, materials, supplies to fulfill requirement stated in the attached Statemetn of Work (SOW). The contractor will be responsible for coordinating and ensuring the effective and economical performance and completion of service calls. See Attached Statement of Work for details. This project is unrestricted, and the North American Industry Classification System (NAICS) Code: 561410, Building Equipment and Other Machinery Installation Contractors; the Standard Industrial Code (SIC) Code: 7338; This is a combined synopsis/solicitation for commercial services/commodities prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice and is issued as a Request for Quote. The solicitation is being conducted under Simplified Acquisition Procedures (SAP) FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24 and Air Force Acquisition Circular (AFAC) 2004-0804. The following attachments are incorporated as part of this request for quote: Statement of Work, Wide Area Workflow Instructions for Electronic Invoicing, and the Department of Labor?s Wage Determination for Pulaski County, Arkansas. Warranty: Standard commercial warranty. The Government reserves the right to evaluate technical compliance/delivery capability and make a best value decision. This could result in award to other than the lowest price offer. Quotes must be received no later than by close of business, 4:00 PM CST, 24 September 2004 and may be received by FAX or e-mail to Points of Contracts as listed on this notice not later than this notice?s response date. STATEMENT OF WORK & DESCRIPTION OF SERVICES 1. SCOPE OF WORK. The Contractor shall provide medical transcription services of outpatient medical services provided to Government beneficiaries at the 314th Medical Group, herein referred to as the 314th MDG or the MTF (Medical Treatment Facility). The Contractor shall provide services in Contractor-supplied facilities. Contractor services include furnishing all labor, reports, transcription of dictation, equipment, transportation and consultation, except as provided in Section C-3. Transcription will be provided for Family Practice, Pediatrics, Specialty, Extended Hours Clinic, Flight Medicine, Radiology, and other outpatient clinics. Performance shall be according to the requirements contained in this Statement of Work (SOW). 1.1. The Contractor shall provide medical transcription services for 314th Medical Group (314 MDG), Little Rock AFB, AR. The Contractor shall provide services at the Contractor?s facility. 1.1.1. Contractor shall provide medical transcription services from dictation (e.g., voice recordings, hand written notes, or typed drafts), which covers a wide range of medical specialties. The Contractor shall transcribe all dictation in the correct Government required format as specified by this contract. The Contractor shall transcribe directly into an automated information system and return the finished product to 314 MDG. The Contractor shall transcribe directly into the Composite Health Care System (CHCS) when required at the 314 MDG. 1.1.2. The Contractor shall produce transcription from hand written notes, typed drafts, or voice recordings. Transcriptions shall be produced in prescribed formats. Contact POC for further details.
