Description

This is a combined synopsis and solicitation for the following commercial item: Automated Document Control Scan System, Mfg: Norcom, Part number: AISDC-SYS-20B or equal. System shall include: Document Control Software; Scan License; Scan Station Co mputer System; Fujitsu (or equal) ScanPartner fi-4120c color/monochrome scanner/25 PPM, with auto sheet feeder; and 90 (ninety) days of software maintenance, upgrades and technical support, or equivalents. This synopsis and solicitation is prepared in a ccordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solici tation is issued as a request for proposal (RFP). Submit written offers (oral offers will not be accepted) via mail, email or fax. All Statement of Need and Evaluation Factor requirements must be met as they appear in this notice. This procurement is being issued as 100% SMALL BUSINESS SET-ASIDE. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13. North American Industry Classification Syst em (NAICS) code 541511 or 541512 with a small business size standard of $21 million applies to this procurement. FOB: Destination for delivery to 5323 East McKinley Ave, Fresno, CA 93727. The provisions at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this solicitation. The provisions at FAR 52.212-2, Evaluation-- Commercial Items, applies to this solicitation. The clause at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation: FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-3 7, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-42, Statement of Equivalent Rates for Federal Hires 52.222-48. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. The clause at FAR 52.224-1, Privacy Act Notification and FAR 52.224-2, Privacy Act, applies to this solicitation. The provision at FAR 52.215-1, Instructions to Of ferors--Competitive Acquisitions applies to this solicitation. Award will be based on technical capability and past performance equal to lowest price, responsible offer. The Government intends to evaluate and award without discussions on this procurement. All proposals must be mailed to Base Contracting Office, 144 FW/LGC, 5323 E. McKinley Ave, Fresno, CA 93727, or faxed to (559) 454-5107. Proposals are required to be received no later than 12:00 PM PST, Friday September 24, 2004. A period of acceptance must be indicated on the proposal and must be good for 30 days.
