Description

Federal Acquisition Regulation 12 (Acquisition of Commercial Items) and with FAR Subpart 13 (Simplified Acquisition Procedures). This is a small business set aside. Small business set-aside is 500 employees. Request for Quotation (RFQ) 673-178-04 is assigned and shall be reference on any request for solicitation. North American Industry Classification System is 333921. The requirement is brand name or equal in accordance with VAAR Clause 852.211-77. Delivery to be made to James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, Florida 33612. Provide By Price the Following: Four (4) Old Wheelchair/Litter Vans to 4 New Vans, Tag Number 43-08264 To 43-1346A, 43-08265 To 43-1347A, 43-21499 to 43-1349A & 43-08266 to 43-1348A. Chevy 3500, 2004 express van. Hour cost to remove wheelchair lift, Med/02 box, jump seat, stretcher tike-down equipment, cargo net, fire extinguisher, tie-down belts, etc. from old van. Remove existing rear bench seating, floor matting & seat brackets from new van provide/install plywood sub-flooring & non-skid vinyl flooring from rear of front seats back. Provide/install recessed tie-down tracks for use w/tie-down belts removed from old van. Provid/install 24? raised roof & roof support system, finished to match van. Provide/install raised side cargo doors, finished to match van. Hour cost to install customer furnished wheelchair lift removed from old van. Remove existing side cargo door panels & replace w/mat material. Hour cost to install customer furnished jump seat removed from old van. Install customer furnished med/02 box removed from old van. Hour cost to install customer furnished stretcher tie-down equipment. Removed from old van. Install customer furnished cargo net & fire ext. removed from old van. Provide/install interior modifications for raised roof/raised door area. All Quotes should be in writing and sent to the James A. Haley VA Hospital, Routing Symbol (90C), 13000 Bruce B.
