- DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with the Statement of Work (SOW) included in this notice. This announcement constitutes the only solicitation, proposal is being requested and a written solicitation will not be issued. THIS IS A SOLE SOURCE REQUIREMENT for a task order to be issued to Missoula Children?s Theater (MCT) , Missoula, MT 59802, to provide CHILDREN AND YOUTH PERFORMING ARTS PROGRAMS at various USAFE Community Center Programs on overseas locations. 1. The Solicitation/Reference Number: F9HQSV42470400 issued as a Request for Quotation. 2. The Set-aside Program is not applicable. 3. Period of Performance: Effective 28 Sep 04 with program performance 20 Jun 05?26 Aug 05. 4. Item 0001: Purchase of non-personal services to provide all labor, materials and transportation to develop Theater camps including arts curriculum for overseas locations stated below, travel to various sites to perform auditions, intensive rehearsals, workshops and finished performances for the public. Production shall come with complete professionally designed and executed scenery, costumes, props and make-up, sound and lighthing equipment, (for approx 630 youths on-stage) in accordance with (IAW) SOW in para 5 below. Two performances are given at end of the week and a minimum of three enrichment workshops will take place outside the rehearsal schedule. 5. SOW for CHILDREN AND YOUTH PERFORMING ARTS PROGRAMS: The contractor shall provide non-personal services to include all labor, materials, transportation, services necessary to develop a new Missoula Children?s Theatre Production for the Performing Arts Program for children and youth ages 5 to 18, specifically for overseas location and conduct 14 Performing Arts Shows at various United States Air Forces in Europe for up to 60 youth per week at bases listed below for youth ages 5 to 18. 5.1. Locations for the one week session as follows: United Kingdom (UK) RAF Croughton, Alconbury, Mildenhall, Lakenheath, Menwith Hill, and Fairford; The Azores, Lajes Air Base (AB); Spain, Moron AB; Germany, Geilenkirchen, Ramstein (2 separate weeks) and Spangdahlem; Italy, Aviano AB; Turkey, Ankara, and Incirlik. 5.2. Operational Requirement and Specific Tasks: The Performing Arts Program focuses on developing youth and community interest in the performing arts through weeklong rehearsals and workshops culminating in performances on the final day for the community. The Reader?s Theatre incorporates staff training and coaching on skills necessary for small performances by children and youth. -- This program concentrates on the development of literacy and life skills in children through participation in the performing arts through reading, creativity, social skills, enhanced self- esteem, goal achievement, and communication skills. Program shall support children and youth of deployed active duty, guard and reserve at all USAFE bases listed above. --- Shows shall be planned and conducted weekly. Two performances are given to the community at the end of the week and a minimum of three enrichment workshops take place outside of the rehearsal schedule for each show. -- MCT shall pick up MCT Production Equipment at the designated storage sites in USAFE prior to their first camp and provide return transportation for equipment at the end of the season. 5.3. Government Furnished Property: The government will provide: -- two rehearsal spaces, a performance space and a piano at each site -- complete registration of participants to include a list of attendees -- each individual contractor instructor shall be provided a Letter of Invitation (LOI)/travel orders -- background checks on all instructors performing in this program. The government reserves the right to disqualify instructors based on results of background checks. 5.4. Professional Requirement of Contractor: -- operates a performing arts camp off host site for at least past 3 years. -- experience with conducting performing arts camp for youth in more than one state and preferably, has received recognition from at least one national organization (i.e Boys & Girls Clubs of America, 4-H, Boy Scouts, Girls Scouts). -- all instructors shall have complete background checks prior to conducting performing arts camp. 5.5. Process and procedures for performing Duties/Performing Arts Program: -Deliverable-Not Later Than 29 Sep 04: New Title, curriculum plan, developmental objectives and program activities for delivery to HQ USAFE/A7SPCY, Unit 3050, Box 160, APO AE 09136, Attn: Ms. Lori Anderson, Dr. Kimberly B. Moore or Jan Seachris. Based on the plan developed, execution of the performing arts program shall be on six selected bases in Jun-Aug 05. -- Logistical requirements shall be coordinated with host site project officers once MCT and HQ USAFE/A7SPCY determine the summer 05 schedule based on the curriculum provided by the contractor NLT 29 Sep 04. This includes 6 specific weekly schedule with times and location. Options for the 14 shows shall be chosen from the bases outlined above. -- Travel itineraries shall be coordinated through HQ USAFE/A7SPCY, Unit 3050 Box 160, APO AE 09136. All expenses shall be the contractor?s responsibility. ADDITIONAL CLAUSE APPLICABLE TO LAJES (PORTUGAL) - AZORES REQUIRED INSURANCE: con a. The Contractor shall procure and maintain during the entire period of performance under this tract the following minimum insurance: As required by Portuguese Law 1) Insurance applicable to civil construction. 2) Portuguese Law #2127, Labor Accidents 3 Aug 65 3) Automobile liability b. The Contractor shall also procure and maintain during the period of performance under this contract insurance to cover injuries to persons or damage to property that may occur in Portuguese territory as a result of acts or omissions done in the performance of duty by their employees. Reference FAR 28.306 ?Insurance under Fixed-Price Contracts? the contractor shall, at its own expense, procure and thereafter maintain the following kinds of insurance with respect to performance under the contract. c. Workmen?s compensation and Employers Liability Insurance as required by law except that if this contract is to be performed in a State which does not require or permit private insurance, then compliance with the statutory or required Workmen?s Compensation insurance shall extent to cover employer?s liability for accidental bodily injury or death and for occupational disease with a minimum liability limit of ?As required by Portuguese Law?. d. General Liability Insurance. Bodily injury liability insurance, in the minimum limits of ?As required by Portuguese Law? per occurrence shall be required on the comprehensive for of policy. e. Automobile Liability Insurance. This insurance shall be required of the comprehensive for of policy and shall provide bodily injury liability and property damage liability covering the operation of all automobiles used in connection with the performance of the contract. At least the minimum limit of ?As required by the Portuguese Law? per person and ?As required by the Portuguese Law? per occurrence for bodily injury and ?As required by the Portuguese Law? per occurrence for property damage shall be required. EMPLOYEE RECRUITING (PORTUGUESE NATIONAL): It is the Contractor?s responsibility to comply with the following requirements for Portuguese National employees. a. The Contractor, using his own forms, may contract directly with, or request assistance from the Civilian Personnel Office, civilian recruitment section, Headquarters Azores Air Command (SRPC, HAAZ), in contracting Portuguese National employees. The Contractor?s contract form must include, as a minimum, the following: name, social welfare number, date and place of birth, residence (village and council), parent?s name, marital status, spouse?s maiden name, professional degree or skill, salary, meal and transportation allowances. b. It will be the Contractor?s responsibility once he has obtained his employees, to clear them through SRPC. c. The Contractor shall request of SPEC/HAAZ forms the number of passes needed. A pass will be issued to each employee. The employee will proceed with the pass directly to the Portuguese Air Police Identification Section as the main gate for proper clearance and return to SRPC/HAAZ to have the pass laminated. d. The Contractor shall inform SPRC/HAAZ when the contract period of performance is changed and return all passes to SPRC/HAAZ upon completion of the contract. e. Failure to comply with paragraph (b) or (c) of this clause may result in the denial of entrance to this installation. f. U.S. citizens and Third Nation personnel, who are residents of Terceira Island at the time they are hired by the Contractor, must comply with the Portuguese Law regarding work permits. It is the responsibility of the Contractor to insure compliance with these laws. Failure to do so may result in the employee being denied access to Lajes Field. 5.6.
