- Provide Community Based Home Care, Skilled, Homemaker, and Home Health Aid Services for beneficiaries of the VA Salt Lake City Health Care System. Offers will be evaluated from contractors who can provide service in the following state: Idaho. Must be Medicare Certified. Must have licensed RN as Director of Nursing. Must have a licensed RN taking home health orders. Priority will be given to non-profit home care providers. Contract will run for one year from date of award with four one-year options to extend. This solicitation is subject to the availability of funds. Issue date approximately September 28,2004. Closing date approximately October 27, 2004.
