- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation reference is FA4800-04-T-0028 is issued as a Commercial Solicitation with Request for Quotation (RFQ) (iii) This solicitation document and incorporated provisions and clauses are those that are in effect through Federal Acquisition Circular 2001-22. (iv) This is a Total Small-Business Set-Aside with NAICS code 562112 with a size standard, averaged over a one-year period of $10,500,000.00. (v) The 1st Medical Dental Support Squadron (1 MDSS), Langley AFB, VA has a requirement for medical waste collection and disposal services to include a base year plus four (4) one year options. This includes the following line items: CLIN 0001-Medical Waste Collection and Disposal Services IAW Statement of Work (SOW) Base Year, period of performance 01 Oct 2004 to 30 Sept 2005. CLIN 1001 Medical Waste Collection and Disposal Services IAW Statement of Work, Option Year 1, period of performance 01 Oct 2005 to 30 Sept 2006. CLIN 2001 Medical Waste Collection and Disposal Services IAW Statement of Work, Option Year 2, period of performance 01 Oct 2006 to 30 Sept 2007. CLIN 3001 Medical Waste Collection and Disposal Services IAW Statement of Work, Option Year 3, period of performance 01 Oct 2007 to 30 Sept 2008 CLIN 4001 Medical Waste Collection and Disposal Services IAW Statement of Work, Option Year 4, period of performance 01 Oct 2008 to 30 Sept 2009 (vi) Description of basic requirements to be acquired: All CLINS are a monthly cost per pound of an estimated monthly requirement of 5,000 lbs of cultures, stocks, pathological wastes, human blood and blood products, sharps, isolation wastes, and outdated drugs. The cost is to include all contractor related costs to include all personnel, training, equipment, tools, materials, supervisions, transportation and other items and services necessary to collect, transport, and dispose of medical waste. Medical waste to be collected and disposed of in accordance with all federal, state, and local requirements and those provisions as stated in the SOW (vii) The above-mentioned medical items will be picked up for transport from Langley AFB, VA 23665 as described in the Statement of Work. (viii) The provision at FAR 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. (ix) The Government will award a firm-fixed price purchase order under this solicitation based upon ability to meet a) technical requirements in the Statement of Work b) price and c) past performance. Therefore, all bidders must submit their best capabilities. Offerors must submit a signed copy of the Statement of Work acknowledging acceptance of the SOW terms and conditions. Bidders must demonstrate a minimum of two years recent experience in collecting and disposing of medical waste. Recent being defined has having completed medical waste collection and disposal experience in the past two years (Oct 2002 to Sept 2004). Offers must submit a list of recent contracts including complete point of contact information where past performance has been demonstrated. (x) Offerors are reminded to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items with its offer.
