- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0066 is issued as a request for quotation (RFQ). Closing Date: September 22, 2004 @ 3:00 p.m. Pacific Standard Time. Offeror will be required to deliver on scheduled delivery dates, and call the government POC prior to expected delivery schedule for required items. CLIN 0001: 10 Kit $___ = $___, Rapid fFN Cassette Kit, P/N: ____ 26 cassettes, filter, tubes 5 kits per delivery. The self lide of the Rapid fFN Cassette is 12 ? 18 months from the date of manufacture. CLIN 0002: 2 Kit $___ = $___, Rapid fFN Control Kit, P/N: ____ . The Rapid fFN Control kit includes a liquid Positive Control (1.2ml/red bottle) and a liquid Negative Control (1.2 ml/blue bottle). These contols have an expiration date of 12 weeks once opened. CLIN 0003: 10 Kit $___ = $___, Specimen Collection Kit, P/N: ____. The Specimen collection kits contains 8 sterile dacron swabs and 8 tubes with a buffer azide solution. One specimen collection enough for 8 patients samples. CLIN 0004: 1 Each $___ = $___, Printer Label, P/N: ____, Printer labels are to print out the Tki IQ patient and QC results. Printer labels roll contains 400 stickers. 1 ea per delivery. Training Assistance: Contractor shall provide to the government with proper training and instruction necessary to begin Rapid Fetal Fibronectin testing, for laboratory, this includes assistance with training of laboratory staff and hands on assistance throughout the validation & clinical correlation process. Procedures for the TLi IQ System are provided on disk and hard copy. Training on proper sample collection will also be provided to physicians, labor & delivery staffs, clinics and other healthcare professionals to insure sample integrity. Training includes a review of patient qualifications and contraindications for Fetal Fibronectin testing. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-24.
