Description

This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation documents; quotations are being requested and no written solicitation will be issued. Responses must contain sufficient documentation to establish a bonafide capability to fulfill the requirements. The solicitation number is 549-145-04 and is issued as a Request for Quotation. The solicitation documents and incorporated provisions and clauses are those in effect through FAC 97-18. The solicitation incorporates by reference FAR Clauses and Provisions 52-212-1,52-212-2,52.212-3, 52-212-4, 52-212-5 and addenda 52.211-6, 52-214-21 and VAAR 852.219-70. The full text of a solicitation clause or provision may be accessed electronically at this/these address (es): http://www.arnet.gov/far/facframe.html. Quotations are due no later than 12:30pm on September 23, 2004. No facsimile copies accepted. Interested parties who are able to provide the requested item(s) should submit sealed quotations on company letterhead. All quotes must meet the minimum listed requirements in this announcement. A completed copy of 52-212-3 and 852.219-70 must be included with quotation. Send completed Quotations referring the solicitation number with attention to Contracting Officer to: VA North Texas Health Care System located at 4500 S. Lancaster Rd., Dallas, TX 75216, here in after referred to as VANTHCS anticipate the sole source award of the purchase of EM-Electronic Media, Micromedex Health Care Series _ 1 year subscription from 10-1-04 through 9-30-05 for the VANTHC Library Service. Offeros must be able to provide the requirements in this announcement. A completed copy of 52-212-3 and 852-219-70 must be included with quotation.
