- SOLICITATION FA8601-04-T-0099 INSTALL GYMNASIUM FLOOR The Operational Contracting Squadron (88 CONS/PKS) at Wright Patterson Air Force Base, OH, is procuring the installation of gymnasium floor, volleyball court, and badminton court. Written quotes are due to the government NLT 1600 hrs on 15 Sep 04. This acquisition will include delivery and installation of all necessary services/materials to meet the government's requirements. A Statement of Work is attached. This requirement will be competed using a written Request for Quotation (RFQ). The NAICS Code is 321999 with a size standard of 500 employees. Offerors' quotes shall be valid for a period of no less than 90 days. The Government reserves the right not to make any award. A complete description of services/materials to be provided is required. The Government requires a sample of flooring material of sufficient size to demonstrate compliance with the required standards to be submitted with the quotation. Resultant contract will have the following structure. All quotes must be broken down and prices shown for each Contract Line Item Number (CLIN) below: CLIN 0001: Replace and repair basketball court, vollyball court, and badminton court. Completed task includes new threshold plates and painted keys. CLIN 0002: Volley Ball equipment: One (1) pair PVBS150 floor sleeves ad cover plates One (1) pair PVB-13U adjustable aluminum standards One (1) PVN-6 net One (1) set VBA Antenae One (1) VBWR Wall Rack This Posting/Synopsis will be the only announcement regarding this acquisition. All interested parties must provide the Government a quote by the time specified above in order to be considered for award under this acquisition. Solicitation provision at FAR 52.212-1, Instructions to Offerors - Commercial (Jan 2004) is hereby incorporated by reference. Award will be based upon best value to the government considering cost and other factors, see FAR 52.212-2 - Evaluation - Commercial Items (Jan 1999). The following factors shall be used to evaluate offers: (1) price, (2) past performance. All offerors shall include with their offers a completed copy of provision 52.212-3, Offeror Representations and Certification Commercial Items (May 2004) (attached). Offerors must also include three references with their offers (include names and phone numbers). The Government may contact any or all of the references for past performance information. This information may then be used in determining who will be awarded any resultant contract. Solicitation provision at FAR 52.237-1, Site Visit, is hereby incorporated by reference. A site visit is scheduled for 8 Sep 04 at 0900 hours. All interested parties must notify Mary Piper by 3 Sep 04 that they will be attending the site visit. The names and Social Security Numbers of persons attending must be provided (limit 2 persons per company). Attendees will meet in the Hope Hotel and Conference Center Parking Lot inside Gate 12A, Wright- Patterson AFB, Ohio, at 0845 hours. Please assemble by the canopy labled "Conference Center". From there transportation will be provided to the gymnasium where the floor will be installed. Section 1008 of the National Defense Authorization Act of Fiscal Year 2001 requires any claims for payments (invoices) under DoD contract to be submitted in electronic form.
