Description

This is a combined synopsis/solicitation for the following BRAND NAME ONLY items: 1) 50ea KaVo 635B Mira Lux 3 Handpiece (MFR: KaVo America; P/N: 553-220); 2) 50ea KaVo 2320LN Contra Angle with integrated motor (MFR: KaVo America; P/N: 535-6108); 3) 50ea KaVo 68LH Latch Head (MFR: KaVo America; P/N: 540-2040); 4) 12ea KaVo 465LRN Multiflex LUX Coupler (MFR: KaVo America; P/N: 553-1550); 5) 12ea KaVo Multiflex LUX Coupler (MFR: KaVo America; P/N: 553-1390). The following numbered note applies to this acquisition: 22. This is a Brand Name Only request. This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. The proposed contract is 100% set aside for small business concerns. This announcement constitutes the only solicitation, and a writen solicitation will not be issued. The following clauses are applicable to subject solicitation: FAR 52.212-1 (Instructions to Offerors), 52.212-2 (Evaluation-Commercial Items), 52.212-3 (Offeror Representations and Certifications - Commercial items), 52.212-4 (Contract Terms and Conditions - Commercial Items), and 52.212-5 (Contract Terms and Conditions Required To Implement Stautes or Executive Orders - Commercial Items). In accordance with clause 52.212-2, the evaluation factors shall be: price and price related factors. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) at http://www.ccr.gov/ to be considered for award. Submit written offers only, oral offers will not be accepted.
