Description
COMBINED SYNOPSIS/SOLICITATION: Base Advisor (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for proposal (RFP). Submit written proposal (to include price quote for all CLINS, base and option years) on RFP reference number FA2517-04-T-6007. (III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. (IV) This solicitation is a small business set-aside; the associated NAICS code is 611710, and small business size standard is $6M. (V) This combined synopsis/solicitation is for the following commercial item: CLIN 0001- Advisor. Advises on all aspects of the Community College of the Air Force associate of applied science program. Assist students with required paperwork. Assists with CCAF graduation preparation. This would entail tasks such as developing a list based on CCAF's recommendation, serving as usher, assisting in sending out notification letters, etc. Approximately one (1) each. See SOW. Base Year Period of Performance 01 Oct 04-30 Sep 05. The CLIN structure represented above (three CLINS each year) will be the same for the four following option years: 01 Oct 05-30 Sep 06 Option Year 1 (One) 01 Oct 06-30 Sep 07 Option Year 2 (Two) 01 Oct 07-30 Sep 08 Option Year 3 (Three) 01 Oct 08-30 Sep 09 Option Year 4 (Four) (VI) Description of requirements are as follows: 1. DESCRIPTION OF WORK. a. A minimum service for one unit will be the conduct of advisement sessions and other duties described in paragraph 2 (Service Delivery Summary) below. The education services officer (ESO) is responsible for assuring that appointments for advisement are scheduled for the contractor. A unit of service will consist of a four-hour block of time mutually agreed on between the education services officer and the contractor. If this cannot be resolved, the contracting officer will make the final decision. b. A standing call for services is hereby established as Monday through Friday, excluding legal federal holidays. The starting time each day will be established by the ESO based on requirements (between 0700? 1700). If there is a conflict on the time for a unit, the situation will be referred to the contracting Officer for resolution. This standing call is adjustable through mutual agreement between the contractor and the ESO. c. The education services officer will provide at least 24 hours notice to the contractor if services are not required on a particular day. Except in the case of unexpected illness or other emergency, the contractor will provide 24 hours notice of inability to provide service. Such instances are expected not to exceed 3 absences during the course of the contract. Individual will not be paid for referenced absences. d. Contractor will be provided access to the base (if required), and the Government will provide all necessary equipment to conduct the work. Government provided equipment includes, but is not limited to, furnished office space, computer hardware and software to fulfill the duties, office supplies and equipment. 2. SERVICE DELIVERY SUMMARY. a. Advises on all aspects of the Community College of the Air Force (CCAF) associate of applied science program. Assist students with required paperwork. Assists with CCAF graduation preparation. This would entail tasks such as developing a list based on CCAF?s recommendation, serving as usher, assisting in sending out notification letters, etc. b. Advises personnel on the Defense Activity for Non-Traditional Support (DANTES) examination program and independent study courses available through DANTES. d. Advises and assists military personnel desiring to attend college with course selection and preparation of AF Form 1227 (Tuition Assistance Request). e. Serves as general education advisor providing information on all educational programs in the area ranging from high school through graduate level. f. Advises active duty members on commissioning opportunities and assists clients in preparation of application packages. g. Provides updates to education office staff members concerning policy changes to education or training programs such as CCAF, ECI, Professional Military Education (PME) schools, commissioning programs, etc. h. Responsible for any minor typing (word processing) and record filing associated with the above duties. i. Contractor will submit invoices on a monthly basis for the portion of the services rendered for the period. The education services officer will determine the format and any attachments necessary to document services. 3. JOB Qualifications. a. Contractor shall have a bachelor?s degree (in Education or a related subject matter area) from a regionally accredited college or university, as evidenced by transcripts. b. It is preferable that the contractor has at least six (6) months experience in a military educational setting. (VII) Period of Performance is 01 Oct 2004 - 30 Sep 2009 (Basic Contract year plus Four Option Years) for service at the following address: 21 SW/HC 455 VINCENT ST, BLDG 1410 PETERSON AFB, CO 80914 (VIII) FAR 52.212-1, Instructions to Offerors ? Commercial Items (Jan 2004), applies to this acquisition. In addition to written quotes for basic and all option years, offerors are instructed to provide a capability statement describing qualifications to perform requirements listed at paragraphs V and VI. (IX) FAR 52.212-2, Evaluation -- Commercial Items (Jan 1999), applies to this acquisition. Paragraph A of this clause is completed as follows: 1) Price. 2) Capability Statement (Pass/Fail Assessment) shall be used to evaluate offers. Only those offerors receiving a ?pass? assessment will be considered for award. (X) The offeror must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (May 2004). A copy of this provision is located at http://farsite.hill.af.mil/ (XI) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2003), applies to this acquisition. (XII) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Jun 2004). Additional FAR 52.212-5 clauses cited in the clause applicable to this acquisition are (b) (5)(i) (14) (16) (17) (25) and (30), (c) (1) and (2). (XIII) The following clauses are also applicable to this acquisition: 52.204-7, Contract Terms and Conditions ? Commercial Items; 52.252-2, Clauses Incorporated by Reference; 252.204-7004 Alt A, Required Central Contractor Registration; 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items; including 252.225-7001, Buy American Act and Balance of Payments Program, and 252.232-7003, Electronic Submission of Payment Requests. (XIV) The Government intends to award a single Firm Fixed Priced contract resulting from this solicitation.
