- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and NO WRITTEN REQUEST FOR QUOTATION WILL BE ISSUED. The solicitation number for this Request for Quotation is W808LN42452400. This solicitation is issued on an unrestricted basis under NAICS number 424210. DESCRIPTION: Eighteen tho usand (18,000) each Prilosec OTC 42-count package. No substitutes will be accepted. Delivery to Madigan Army Medical Center Pharmacy, Tacoma Washington 98431 of three thousand (3,000) packages per month for a six month period. First delivery no later th an 30 days from acceptance of award. Offers responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Officer-Commercial Items. As stated in this clause, the Gover nment intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial proposal should contain the best terms from a price and availability standpoint. In accordance with FAR 52.212-3, evaluation factors are: price and availability. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision FAR 52.212-3, Offeror Representations and Certifications Commercial Items . An authorized representative of the offeror must sign these representations and certifications; the offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) , and Tax Identification number (as required). The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items . FAR Clause 52.212-5, Contract Terms and Conditions Req uired to Implement Statutes or Executive Orders Commercial Items (clauses 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33) and DFAR Clause 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (clauses 52.225-7001 and 52.232-7003) apply to this acquisition.
