Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12 (Acquisition of Commercial Items) and with FAR Subpart 13 (Simplified Acquisition Procedures). Request for Quotation (RFQ) 673-187-04 is assigned and shall be referenced on any request for solicitation. North American Industry Classification System is 334510. The requirement is brand name or equal in accordance with VAAR clause 852.211-77. Small business size standard is 500 employees. Delivery to be made to Brevard Outpatient Clinic, 2900 Veterans Way, Viera, Florida 32940. Provide by price the following: One (1) RS-253 Acoustic Systems Sound Room with one (1) extra power outlet, one (1) Conduit drop and one (1) Oversized window 30? X 36?. To be installed on the ground floor. One (1) Verifit VF-1 Real Ear/Hearing Verification System; Clinical with Monitor Headset, 4 battery pills, and Record transducer Audioscan. One (1) GSI Audera DPOAE Otoacoustic Emission Testing System only with Laptop Computer. Provide shipping and installation charges. Provisions and clauses found at FAR 52.212-1; 52.212-4 and 52212-5.
