Description
This is a combined synopsis and solicitation for the following commercial items: Contractor shall provide all labor, tools, materials, equipment, facilities, transportation and all other items as necessary to provide laboratory services as specified. Lab Test pick-up and delivery at Buliding 61, room 93, 52 Medical Group Hospital Laboratory, Bitburg AB, Germany. Specimens to be picked up daily between the hours of 0900-1400, Monday thru Friday excluding German Holidays. Speciments being shipped to the contractor laboratory are to arrive within 24 hours of pick up. Transportaion conditions must ensure no specimen deterioration occurs. Contractor will provide results via the internet and will fax any critical results to 065618492. Reports shall meet all College of American Pathologists (CAP) requirements and suitable for reproduction in a Xerox type machine. The contractor must render test results within seven (7) workdays of receipt of the specimen. The contracted laboratory must be comprehensive and able to persorm as a minimum, all the tests in their guidebook. Results must correlate with the clinical picture presented by the patient. Repeat analysis, at no charge to the governement must be provided on request in the event of a clinical disagreement. To provide this sevice, all specimens must be retained at leat two weeks. The contractor must be accredited by the College of American Pathologists (CAP) and participate in the CAP Surveys programs. Quarterly results of the CAP surveys must be provided upon request at no charge to the government. Documentation of in house quality assurance programs and data shall be provided to the 52d Medical Group Hospital Laboratory upon request at no charge to the government. The contractor shall provide, at no charge to the government, tests on split samples for method evaluation. The 52d Medical Group Hospital Laboratory must notify the contractor in advance to identify these specimens. Technical consultation services shall be available to discuss results and their significance with the 52d Medical Group Hospital staff. The service will be at no charge to the government and available to all clinicians and laboratory staff. The contractor shall provide copies of their ?Laboratory Guide? giving normal ranges, specimen requirements, and result significance at no cost to the government. Lists of set-up times shall be provided upon request at no charge to the government. Special shipping containers and/or special supplies shall be provided at no cost to the government. In the event the contractor receives an unacceptable specimen, the 52d Medical Group Laboratory must be notified within 24 hours via telephone. The contractor must agree to notify the 52d Medical Group Hospital Laboratory of any price increase at least 60 days prior to the increase. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. North American Industrial Classification Standard 621511 applies to this procurement. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F53SGL42580007. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13.
