- This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6., as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; requests for quotes are being requested and a written solicitation will not be issued. This requirement is for All or Nothing. Reference number: RFQ 673-90F-04-0053. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-24. NAICS code is 421450, 500 Employees. This solicitation is 100% set aside for small business. James A. Haley VA Hospital has a requirement for the following System: DESCRIPTION: BRAND NAME ONLY. One (1) each, TOPCON TRC-NW200 Camera with DVS System, ALT-15 Table and LCD Monitor; One (1) each, Haag-Streit 900 BQ Slit Lamp, Table Model; One (1) each, Lombart DVS System with Haag-Streit Video, Sony DXC-390 3 Chip Camera and Background ILL; One (1) each, Heine Video Omega 2C Set Complete with Accessories. Place of delivery is 59th MDW/MSLE, 2200 Bergquist Drive, Suite 1, Lackland AFB, TX 78236. This is for Lackland AFB, Texas. Delivery, which shall be FOB Destination, shall occur 30-45 days after award of the purchase order. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror--Commercial Items. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition.
