Description
The purpose of this amendment is to provide questions and answers and to distribute the revised Statement of Work dated 23 Sep 04. QUESTIONS AND ANSWERS are as follows: QUESTION 1: Is the existing carpet to be removed a broadloom product? ANSWER: No. QUESTION 2: Are we responsible for any furniture moving? If so, what type? i.e. Standard Office, Modular, etc. ANSWER: In Bldg 499, the Contractor is responsible for moving standard office furniture. In Bldg 597, the Contractor is responsible for moving standard office furniture (495 SY) and modular furniture (165 SY). QUESTION 3: Is there any wall base to be installed? If so, what type? i.e. Wood, Rubber, Vinyl, etc. ANSWER: No. QUESTION 4: What are the subfloor types? i.e. Wood or Concrete ANSWER: In Bldg 597, the subfloor type is concrete and raised flooring. In Bldg 499, the subfloor type is concrete. QUESTION 5. Is the vinyl base in Bldg 597 to be replaced? ANSWER: No. QUESTION 6: How much extra stock would you like for Bldg 597 (customary is 5 percent overage)? ANSWER: 50 SY QUESTION 7: Is the toe base in Bldg 499 to be replaced? ANSWER: No. QUESTION 8: Would you like us to replace the toe base in Building 597 and Building 499? This would be the toe base only and would require approximately 1500 linear feet of material. Would you want us to submit this as a separate line item? ANSWER: No. QUESTION 9: The Engineering Technical Letter (paragraph 9.3.6) calls for 10 percent additional material for pattern match. Standard business allowance is 3 percent--please verify what we should use. ANSWER: Use 10 percent. QUESTION 10: What is the allowance for shelf stock for carpet tile for Bldg 597? ANSWER: Assume this is the same as question as Question 6: 50 SY. QUESTION 11: Please verify if I understood the Statement of Work correctly—the Government will supply the carpet for Bldg 499 and the 9 inch border and vinyl door trim for Bldg 597. ANSWER: The Government will ONLY supply the Carpet for Bldg 499. The Contractor is responsible for supplying all other materials/items. Reference revised Statement of Work dated 23 Sep 04 which deletes the 9 inch border all together and changes the vinyl door trim to contractor-supplied. STATEMENT OF WORK, REPLACE CARPET, BLDGS 499 and 597, (Revised 23 Sep 04) is as follows: 1. Scope: Contractor shall provide all labor, tools and equipment to perform the following: Replace carpet, Bldg 499 includes: Remove approximately 1,500 SY of carpet. Dispose of removed carpet properly at an off-base site. Install carpet (Government-furnished 6 foot roll carpet with composite backing by glue-down method. Adhesive furnished by Contractor). Replace Carpet, Bldg 597 includes: Remove approximately 660 SY of carpet. Dispose of removed carpet properly at an off-base site. Install carpet tile by glue-down method (Contractor provided 18 inch X 18 inch carpet tile per specifications below. Adhesive furnished by Contractor). Install approximately 54 linear feet of standard vinyl door trim (Contractor-furnished trim). The color of the trim should match the existing trim. Contractor shall submit product data with a color sample of the vinyl door trim for Government review. 2. Carpet Tile for Bldg 597 shall be 18 inch X 18 inch and meet the specifications of the following brand (brand name or equal): Mfg: Collins and Aikman (C&A) Floorcovering; Luminaire II ER3 Modular Tile; Product Style: 00122; Product Color: 96006. Contractor shall submit product data with a color sample of the carpet tile for Government review. The Contractor should not order the carpet tile before receiving approval from the Contracting Officer. 3. All work will be in accordance with the attached Engineering Technical Letter (ETL) 03-3, dated Apr 16 2003. 4. The work locations are in a restricted area. Contractors working in these areas will have military escorts. Quotes may be e-mailed to Stacy Mazurek at stacy.mazurek@offutt.af.mil or faxed to (402) 294-4069. The quote should reflect a separate price for each building. THE ESTIMATED DELIVERY TIME, INCLUDING INSTALLATION, SHOULD ALSO BE ANNOTATED ON THE VENDOR’S QUOTE. Quotes are due NLT 1600 hours local time on 24 Sep 04. Vendors are advised to monitor this website for any updates and/or changes.
