Description

The Contractor shall provide all labor, materials, transportation and supplies for IV Home Infusion Therapy for the VA Medical Center, 4150 Clement Street, San Francisco, CA 94121 for a period of one year from the date of contract award. The solicitation will be issued o/a September 28, 2004 at the following website: http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa This is a commercial items procurement and the following clauses, provisions and addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions, 52.212-5 Terms and Conditions to Implement Statutes or Executive Orders. The proposals should be mailed to VA Northern California Healthcare System, 150 Muir Road, Martinez, CA 94553, Attn: John Gregorio (90-CCA). No fax or web based proposals shall be accepted. ALL questions MUST be in writing and may be emailed to John.Gregorio2@med.va.gov or faxed to 925/ 372-2044. No phone inquiries will be returned. No question will be answered over the phone. This is an unretrictive procurement. The offeror is responsible for downloading all documents including amendments at the websited listed above, without further notification from the VA.
