Description
Statement of Work - Judges Chambers Remove and replace approximately 3200SF of ceiling tile and replace with new tile. Use existing drop in ceiling grid. dispose of debris. Remove approximately 300 SY oc carpet and dispose of debris Install approximately 300 sy of government furnished carpet. Remove approximately 1200 SF of vinyl wall covering. Deispose of debris. Prep and patch walls as necessary for installation 1200 SF of new vinyl wall covering. Wall covering should be installed per industry standards. Clean all wood paneling/approximately 4000SF. All color and material selections will be approved by government representative. There are approximately 40-50 floor to ceiling bookshelves stocked with several thousand books. Shelves and books must be removed and placed in exact location. Books must also be returned to exact locations. A site visit will be conducted on Oct. 13, 2004 to preview the site. Please conduct Mr. Michael Pettinelli at (518) 378-8860 to schedule a time at this date. You must make a appointment to preview the site. After award, a security clearance shall be obtained by all persons on the contractor?s workforce, including the general contractor foreman, supervisory project manager, all tradespersons and all sub- contractors? labor force. Within 14 days of contract award, the contractor shall have submitted all completed security clearance application documents as follows for those employees designated to work on this project. No work will be performed until all employees have a background clearance. GSA-176, Statement of Personal History - Only the 2002 revision of this form will be accepted. All questions on the form must be answered or the entire package will be rejected by FPS. FD-258 Fingerprint Card (Two original fingerprint cards are required.) ? this form shall be typewritten, with signatures in all designated locations on the forms. If the form is not typewritten with all signatures in designated locations, the entire package will be rejected by FPS. Photocopy of the applicant?s Social Security Card. If this is not included, the entire package will be rejected by FPS. If the applicant was an Immigrant who became a citizen, or is presently an Immigrant with working privileges, s/he must also include photocopies of naturalization or alien paperwork (green card working papers) along with their GSA-176 before clearance can be processed. . Those persons who have cleared security within the last 3 years do not need to complete form GSA-176. However, they do need to submit their name, social security number, photocopy of the Social Security Card, date of birth and completed Form FD-258, Fingerprint Card.
