- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. THIS IS A REQUEST FOR QUOTATION (RFQ) USING SIMPLIFIED ACQUISITION PROCEDURES. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-25. This acquisition is NOT set aside for small businesses. The NAICS code is 511210 and the related small business standard size is $21 million. The Social Security Administration?s Office of Telecommunications and Systems Operations has a requirement for maintenance on Government owned software that is already installed and in use on machines in Wilkes-Barre, PA, 18702. The Contract Line Number (CLIN), quantity, and unit issue of the software to be maintained (maintenance is described in the technical specifications below) are as follows: CLIN 0001 TIS EXPORT/ARCHIEVE STATIONS, five (5) each; CLIN 0002 TIS INPUT STATION (IS-01000), five (5) each; CLIN 0003 TIS KODAK 9500 DRIVER (IS-01050), five (5) each; CLIN 0004 TIS RE-KADMOS REG ENGINES (RE-01090) thirty (30) each; CLIN 0005 TIS SOLUTIONS AEG25 CPS REG ENGINES, thirty (30) each; CLIN 0006 TIS SOLUTIONS CKG 29 CPS REG ENGINES, thirty (30) each; CLIN 0007 TLS SOLUTIONS PROCESSING STATION, thirty (30) each; CLIN 0008 TIS WORKFLOW MANAGER, (WM-01000), five (5) each. The following are the technical specifications of the maintenance: must include updates, upgrades, and fixes. The vendor shall deliver all items electronically, or FOB Destination to Social Security Administration, Wilkes Barre, DOC, 1150 East Mountain Drive, Wilkes Barre, PA, 18702. The period of performance for maintenance is 11/10/04-11/9/05. The provisions at 52.212-1, Instructions to Offerors-Commercial Items, and 52.212-2 Evaluation-Commercial Items, apply to this acquisition. The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The clause at 52.212-4 Contract Terms and Conditions?Commercial Items applies to this acquisition. Additionally, the clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items, applies to this acquisition, including subparagraphs (14) 52.222-3; (15) 52.222-19; (16) 52.222-21, (17) 52.222-26, (18) 52.222-35, (19) 52.222-36, (20) 52.222-37, (23) 52.225-3, (25) 52.225-13, and (31) 52.232-34. All offerors shall submit the following: 1) Quotation which addresses maintenance for all CLINs; 2) Description of maintenance, including any telephone number or contact information, and the hours of any telephone support; 3) Copy of most recently published price list(s); 4) A completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items which may be downloaded from http://www.acqnet.gov/far/loadmainre.html.
