Description: The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare service purchase order for OB/GYN consultations, direct patient care and Obstetric/Gynecological educational services at the Fort Thompson Service Unit to include two full or eight-hour days per month for one (1) year at the facility by the contracting physician and 34-hour consultation, when available. The period of performanc will be January 1, 2005 through December 31, 2005 with one option year. This is a Small Business Set-Aside request. Requests for the RFQ package shall be in writing either by telefax or email. The closing date of this RFQ will be December 10, 2004 @ 3:00 p.m. CST.
