Description
This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice for the South Texas Veterans Health Care System. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This procurement is being issued as a Request for Quotations (RFQ), and the reference number is 671-19Q-05. This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2001-26. This is a 100% small business set-aside. The NAICS code is 532490 and the size standard is $6.0 million. The Government anticipates awarding a single firm-fixed price contract to the contractor providing the best value for the Government in regards to specifications, quality, and price, of equipment and maintenance. Contractor is required to provide lease pricing for one (1), 48 volt electrical, 4 wheel sit down, 5,000 lb. lift that meets or exceeds the following specifications: 2-stage mast with 138" raised fork height, 93" overall lowered height, 69" free lift, 42" wide hook type carriage with side shifter, 48" high load backrest, 42" long hook type pallet forks. Power source; 48 volt, solid state SCR control with micro processor control panel, dash display, heavy duty drive and pump motors, Additional Features; self adjusting brakes, regenerative breaking, parking brake, fully filtered hydraulic system, hour meter, battery discharge indicator, full overhead safety guard with safety seat, power steering, diagnostic display, tilt steer column, seat belts, headlights, safety strobe light, back-up alarm, heavy duty industrial battery, heavy duty industrial charger equipped with auto stop/start to prevent overcharging. Lease period will be for base year and 3 option year renewals. All appropriate safety equipment (lights, horn, back-up alarm, etc.) must be in proper working order. Quote must include a preventative maintenance program. This forklift will be utilized in support of an alternate warehouse location for approximately 8 hours per week. Contractor must be able to respond to equipment failure within 4 hours or less. If forklift is deemed inoperable, replacement must occur within 24 hours to ensure forklift availability at the facility. All quotes shall include pricing for F.O.B. destination to VA leased warehouse located in San Antonio, TX. The following FAR clauses and provisions are applicable to this solicitation: ALL OFFERORS ARE TO INCLUDE A COMPLETED COPY OF THE PROVISIONS AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS WITH OFFER. The following FAR clauses and provisions are applicable to this solicitation: 52.212-1, Instructions to Offerors-Commercial Items, 52.212-4, Contract Terms and Conditions--Commercial Items; 52.204-7 Central Contractor Registration; 52.247-34 F.o.b. - Destination, 52.217-9 Option to Extend the Term of the Contract(30 days/60/4 years); 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercials Items; The following FAR clauses identified at paragraph b of 52.212-5 are considered checked and are applicable to this acquisition: (5),(11-20),(24ii),(32) section (c) (1),(2),(3), 52.232-19 Availability of Funds for Next Fiscal Year, 52.219-1 Small Business Program Representations, VAAR 852.219-70 Veteran-owned Small Business, VAAR 852.270-4 Commercial Advertising. Wage Determination Number 1994-2521(Rev 27) applies.
