Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation 673-58-05 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-11. This Solicitation replaces Solicitation Number 673-24-05. This is a commercial item procurement under the North American Industrial Classification System (NAICS) Code 421450, and Small Business Size Standard of $6.0 Million per annum. This Solicitation is a 100% set-aside for small businesses. This solicitation is for 11 Patient Care Modules. Unit dimensions shall be: 40? to 48? wide X 16? to 18? Deep X 35? to 36? high and conform to current ADA standards. Unit shall have: Have a steel or metal frame cabinet. Unit shall have: Solid surface Corian counter top with sink (all one piece) with two (2) 3? (inch) backsplash on back and side. Unit shall have: Built in 316 stainless steel dialysis box with single valve, drain and mixing valve mounted within the stainless steel box. Unit shall have: Sloan 900 Series flush valve or equal, mounted with easy access to service valve. Unit shall have: a 1000lb load rating. Unit shall have: T&S Brass Model goose neck faucet with 4? wrist blade handles and laminar flow nozzle or equal.Unit shall have: removable metal access panel for plumbing components and ease of maintenance. Unit shall have: built-in Chicago faucet bedpan washer mounted and concealed within the cabinet which includes 48? flexible stainless steel hose, self closing spray valve, positive shut off, volume control and elevated pressure type vacuum breaker. Unit shall have: pivotal 1? thick padded toilet cover and padded backrest. Toilet seat height shall meet ADA requirements. All requested components shall be a factory installed. Unit shall hook-up to existing utilities with minimal modifications. Vendor is required to field verify all existing connection, locations and number of left and right units. Units shall be delivered and installed no later than April 4th, 2005. All quotes are to be mailed or fax to Luisa Rodriguez, at the James A. Haley VA Hospital, 13000 Bruce B.
