- The Uniformed Services University of the Health Sciences (USUHS), the Department of Defense Medical School and the Health Sciences University, hereby announces the requirement for a telephone survey to be conducted. Quotes are being requested and a written solicitation will not be issued. Please submit all quotes before the closing date of February 22, 2005. The statement of work is as follows: Period of Performance: Three months, beginning April 1, 2005 and ending June 30, 2005. Description of Project: A telephone survey examining the influence of personal and environmental factors on cancer screening will be conducted. The list of survey questions will be provided. A total of 500 low-income, African American individuals who are over 50 years old, and who use Bureau of Primary Health Care (BPHC) clinics need to be interviewed. The survey will be conducted by a firm specializing in conducting telephone surveys. Telephone numbers will be randomly selected based on a purchased sample list. Upon answering the phone, interviewers will explain to a respondent that he/she was selected to participate in a survey about health care and will ask if the individual would like to participate. If the respondent provides consent to participate, the interviewer will then establish eligibility by asking: 1) if respondent is 50 years or older, 2) if individual uses a BPHC clinic, and 3) and the race/ethnicity of the individual. If the individual is not 50 years or older, the interviewer will ask to speak to someone else in the residence that meets the age eligibility. If respondents meet the age eligibility, but does not use a BPHC clinic or does not self-identify as an African American the telephone survey will be terminated. If there is no answer, a number will be called a total of 6 times before it is discarded and replaced by a new number. Interviews will take place between the hours of 9 a.m. and 8 p.m. daily. Upon completion of data collection, the survey research firm will provide a data file that will not include any identifiers. Task Description: The tasks involved include the following: Task 1. Develop Sampling Procedures (Month 1): A. Construct a sampling procedure. Task 2. Conduct Survey (Months 2 and 3): A. Create data codes and fields. B. Implement survey. Task 3. Obtain Data File from Survey Firm (End of Month 3) A. Provide a data file to the client. Deliverables: Documentation of sampling procedures Participation rate Codebook Data file (in Excel or SPSS). Evaluation Factors : Technical excellence: The contractor should have a proven record of conducting telephone surveys in the area of health research. Past performance: The contractor should have expertise in working with academic institutions and on low-budget, small projects. Price.
