Description

Davis-Monthan Air Force Base, Arizona seeks a Radiology Transcription Service. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Contractor shall provide all equipment, tools, transportation, personnel, labor, supervision, and management necessary to provide radiology transcription services for Government beneficiaries of the Medical Treatment Facility, 355th Medical Group, 4175 S. Alamo Ave, Davis-Monthan AFB, AZ, 85707-4405; place of performance. This requirement will be posted for two days. Details of services and qualifications will be provided in the statement of work. Award will be made based upon best value to the government. Prior training and prior and/or current authorization for Composite Health Care System is preferred. All transcription must be made in the United States--not subcontracted overseas. This notice does not obligate the government to award a contract or to pay for any associated cost to contractors. Solicitation FA4877-05-Q-0323 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24, the RFQ will be issued in a commercial format. This procurement is being issued as a 100% Small Business Set A-Side, under the applicable North American Industry Classification Code 561410. The period of performance is 1 May 2005 ? 30 September 2005, contract start date is on or about 1 May 2005. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price.
