- This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Southeast Region which includes all DoD Medical Treatment Facilities (MTFs) in the states of Alabama, Georgia, Florida, Mississippi, South Carolina, and Tennessee are in the proc ess of standardizing CUSTOM SURGICAL PACKS as listed in the subject line above. Companies who currently possess or are in the process of obtaining a Distribution and Pricing Agreements (DAPAs) with the Defense Supply Center Philadelphia (DSCP) for this p roduct are eligible to participate in the standardization process. Please respond by email to: Virginia.holeman@se.amedd.army.mil . Provide your company name, point of contact, address, phone number, fax, email address, DAPA Number, Solicitation Number a nd answers to the below listed questions no later than 21 days after the posting of this announcement. For questions, please call Ms Ginny Holeman at (706) 787-2019. 1. Can your company provide a full product line of Custom Surgical Packs? 2. Does your company manufacture and/or distribute custom surgical packs? 3. Are your products available through the Prime Vendor, Owens & Minor? What is your Owens Code Number? 4. What is your DAPA Number 5. Under what brand(s) does your company manufacture or distribute your product? 6. Does your company provide utilization reports, sales reports, and pack change reports? 7. What education programs and clinical support for your packs does your company provide? 8. Does your company offer any software programs for pack management that is available to the facilities? If so, what is the program and is it available at no additional cost? If there is a cost, what is it? 9. How does your company clearly label the latex status of your packs? 10. Are there fees for component changes in the packs? If so, what are they? 11. Does your company offer the ability to choose the vendor for components in the packs without penalties or added fees? 12. Is your company amenable to using standardized products chosen by our Region in the packs? If yes, what kind of monitoring tool does your company have to ensure the standardized products are continuously being used in the packs? What kind of feedba ck mechanism can you provide to our Region in this regard 13. Are there fees for the standardized component changes and if so what are they? 14. If your company has its own brand of products, but our Region requests to substitute standardized products, is there a penalty or added fees? If so, what are they? 15. Does your company have a special arrangement with manufacturers/distributors to purchase regionally standardized products at the government negotiated incentive pricing? 16. If asked to use standardized products, are there any companies you do not have a business relationship with? If so, please disclose at this time. 17. At the time of the incentive pricing request, would you upon request provide a cost breakdown between your usual brands of products versus the region specific standardize products? 18. What form of notification does your company provide when the chosen vendor for components is not available? 19. Does your company package packs according to configurations that are facility specific? 20. What is the minimum length of time your company requires for a facility to request discontinuation to use, or make changes to components of a custom pack? 21. What is the average turn around time from placement of the change to a component in the pack to the actual change? 22. What is the method used for listing components contained in the pack? 23. Are expiration dates listed on the label for items in the pack with specific expiration dates (i.e. solutions, gels, etc)? 24. What is the method of sterilization used by your company for the packs? 25. How does your company ship the product to maintain the integrity and sterility? 26. What types of educational tools or materials do you have for this pro duct line? 27. Does your company offer a continuing education program? If so, what is the cost? 28. Can you provide customer support in the form of a sales representative for each and every Medical Treatment Facility (MTF) within the TRICARE Southeast Region? If so, can you provide a list upon request? 29. If your company is awarded the business for our Region, how soon can your company deliver custom packs to the facilities? Do you have a written Implementation Plan? 30. Does your company provide customer service 24/7 through a toll free number? 31. Does your company have local representatives to provide assistance at facility locations? 32. What other services not asked does your company provide at no cost? 33. What other services not asked does your company provide for an additional fee? 34. What is your return goods policy? 35. Where are your warehouses located? 36. Is there any history of backorders and/or recalls for this product group? If yes, please answer: a. Dates and duration b. Cause c.
