Description: It is the intent of the Phoenix Area Indian Health Service to enter into a contract with Indian Walk In Center, Inc. The contractor is to provide limited health care service by referral, health education, and preventative health care services to Native American and Alaskan Natives in the Salt Lake City, Utah area. This is a sole source set-aside requirement under statutory authority P.L. 94-437, Title V, Indian Health Care Improvement Act, as amended. The services will be for a base year with two one-year options to extend. Solicitation will be issued to Indian Walk In Center, Inc.
