Description
This synopsis is for the following commerical requirement. PICKUP AND DISPOSAL OF MEDICAL WASTE, NAVAL HOSPITAL BEAUFORT, SOUTH CAROLINA. The contractor shall supply all labor, materials, equipment, supplies, transportation, and supervision necessary to pickup and dispose of infectious waste for Naval Hospital, Beaufort, South Carolina, including two clinics. The contractor shall supply all labor, materials, equipment, supplies, transportation, and supervision necessary to pickup and dispose of infectious waste for Naval Hospital, Beaufort, South Carolina, in accordance with all federal, state, and local regulations including: a. State transporter permit and ID number, Contractor shall provide a current copy of their state transporter permit and identification number. b. Materials and Supplies: Contractor shall provide all bags, boxes, labels, tape or any other type container used to package infectious waste, at no additional cost to the government. c. Locations and Pickup Schedule: The pickup day shall be the same for all three locations. Locations are: (1)Naval Hospital, Beaufort, 1 Pinckney Boulevard, Port Royal, SC, Building 1. Registration number SC07-0085G, Generates aproximately 20 ? 375 boxes per week, Contractor shall pickup once a week on the day of the week. (2)Marine Corps Air Station, Beaufort, SC, Building 598. Registration number SC07-0095G, Generates about 2 boxes per month-average weight of 40 pounds per box, Contractor shall pickup once every two weeks. (3) Marine Corps Recruit Depot, Parris Island, SC, Building 669, Registration number-SCO7-0096G, Generates about 7 160 pound boxes per week, Contractor shall pickup once a week. d.Documentation: (1) Contractor shall provide an infectious waste manifest form at the time of pickup and follow up with a ?Certification of Incineration? issued by the incinerator facility when the disposal has been completed. Each box must be weighed so there is an accurate total weight on each manifest issued. (2)Contractor shall ensure that the state permit or ID number for each generator, transporter, and treatment facility is entered on the manifest and that each manifest is signed and dated by the generator on the manifest form before the transporter leaves the generator site. This synopsis is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. Any award(s) resulting from this solicitation will be made using the following set-aside order of precedence: (a) In accordance with FAR Subpart 19.13, any award under this solicitation will be made on a competitive basis first to an eligible Historically Underutilized Business Zone (HUBZone) small business concern (see Section K, 52.219-01, ?Small Business Program Representations? for small business size definition) in accordance with FAR Subpart 19.13, provided that there is adequate competition among such firms. (b) If there is inadequate competition for award to a HUBZone small business concern, (a) In accordance with FAR Subpart 19.1201, any award under this solicitation will be made on a competitive basis to an eligible Service Disabled Veteran Owned Small Business (SDVOSB) small business concern (see Section K, 52.219-01, ?Small Business Program Representations? for small business size definition) in accordance with FAR Subpart 19.1201, provided that there is adequate competition among such firms. (c) ) If there is inadequate competition for award to a SDVOSB small business concern, award will be made on the basis of full and open competition considering all offers submitted by responsible business concerns. NOTE: ALL INTERESTED (8a) HUB-ZONE, SERVICE DISABLED VETERAN OWNED SMALL BUSINESS concerns will notify this office via-email of their intention to submit a quote on the proposed contract as prime contractor no later than 01 September 2005. This notification must include name of company telephone number and point of contact. If adequate interest is not received from 8(a), HUB-ZONE or SDVOSB concerns by 1 September 2005, then the procurement will be issued unrestricted. The Government intends to issue the entire solicitation on or about 1 September 2005. The entire solicitation will be available for viewing and downloading at www.esol.navfac.navy.mil.
