- The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide Respiratory Therapist Services for veteran beneficiaries at the VA Medical Center, Milwaukee, WI. beginning October 17, 2005 for a period of three months from the date of the award in accordance with all specifications, work statement, terms and conditions contained therein (with two options to extend for two months each exercised at the government?s discretion). Therapists will be called in on an as needed basis. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Offers, including options, shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. This is a Firm Fixed Price contract and this acquisition is a 100% small business set-aside. The size standard for North American Industry Classification System (NAICS) code 621399 is $6.0 Million. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
