- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W91QF2-06-T-0003 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 05-06 September 30, 2005. This acquisition is being conducted in accordance with FAR 12 and in conjunction with FAR 13 Simplified Acquisition Procedures. This procurement is 100% Set-Aside for Small Business Concerns. The associated NAICS Code is 813110 with a Small Business Size Standard of $6.0. Contract line Item 0001. The project consists of Administrative Assistant/ Religious Coordinator Services for a Base Year and four Option Years. CLIN 0001- 2,000 hrs. of Administrative Assistant/ Religious Coordinator Service for Base Year November 1, 2005- September 30, 2006, CLIN 0002- 2,000 hrs. of Administrative Assistant/ Religious Coordinator Services for Option Year 1 October 1, 2006- September 30, 2007, CLIN 0003- 2,000 hrs. of Administrative Assistant/ Religious Coordinator Ser vices for October 1, 2007 September 30, 2008. CLIN 0004- 2,0000 hrs. of Administrative Assistant/ Religious Coordinator Services for October 1, 2008 September 30, 2009. CLIN 0005- 2,000 hrs. of Administrative Assistant/ Religious Coordinator Services for October 1, 2009 September 30, 2010. Work shall be performed in accordance with the Governments Statement of Work. Description of Requirement - STATEMENT OF WORK (SOW) 1. GENERAL 1.1. Scope of Work. The contractor shall serve as the Religious E ducation Coordinator (REC) for the Fort Hamilton population providing a Catholic and Protestant and Jewish RE program consisting of weekly Confraternity of Christian Doctrine (CCD) and Rites of Christian Initiation for Adults (RCIA) classes as per the dict ates of the Roman Catholic Church, Sunday School Classes, and Jewish Lunch and Learns. He/she shall also provide preparation for the Holy Sacraments, Adult Education, special holy day events, Vacation Bible School (VBS), and volunteer coordination. 1.2. Location. Primary facilities for the services provided will be the Fort Hamilton Post Chapel. The contractor, upon assignment by the Contracting Officer's Representative (COR), shall provide services in any chapel or facility located within the confines of the Federal installation known as Fort Hamilton. 1.3. Workmanship. All work shall be accomplished within the guidelines of the Roman Catholic Church, Army regulations and directives, and the Installation Chaplain's Policy Statements. 1.4. Loss/Damage /Theft. The contractor shall be liable for loss, damage, or theft of Government property due to the contractor's carelessness and/or negligence. Negligence will be determined by appropriate Report of Survey findings. 2. CONTRACTOR PERSONNEL 2.1. Compe tency of Contractor Personnel. All services under this contract are to be performed by an adult of at least 21 years of age with verifiable moral character and spiritual maturity. He/she shall possess at minimum a two year college degree, or have at leas t two years of experience in a military chapel as an REC/Pastoral Assistant or teacher, and have credible references. Applicants must be willing to undergo a background check similar to that required of a childcare provider for U.S. Army Child Care Center s. The contractor shall be fully qualified to perform the required services without any supervision or direction by Government personnel. 2.2 Preferable Skills & Abilities: Work experience with Military Chapel functions and families (a) Certified Notary Public, for clients of the Chapel, and the Military Community (b) Military and Commercial Drivers License, to include driving military bus up to 24 passengers for chapel trips (c) Suicide training, through office personnel (d) Defense Travel System training 2.3 Contractor Points of Contact. The contractor shall provide the Contracting Officer and the COR wi th the names of a primary and alternate point of contact with telephone numbers, in case of emergency. 2.4. Personal Property. The contractor shall ensure that personal property is clearly marked and identified to preclude misidentification as Governme nt property. 3. PERFORMANCE EVALUATION MEETINGS 3.1. The contractor shall meet with the Installation Chaplain once per month. The contractor shall contact the Installation Chaplain to make appointments for this meeting. 3.1.1 The purpose of these meet ings will be to discuss the needs assessment of the Catholic/Protestant/Jewish population located at Fort Hamilton, program evaluation and program activity scheduling. The contractor and/or the Contracting Officer's Representative shall notify the Contrac ting Officer of any discrepancies, disputes, or indications of poor performance. The Installation Chaplain, if not the COR, shall notify the COR of such indications. 4. HOURS OF OPERATION 4.l. Performance. The contractor shall provide the required pr oducts and services as indicated in the delivery schedule of the contract. 4.2 Hours. The work will be accomplished between the hours of 0700-1700 Monday thru Friday. 5. GOVERNMENT-FURNISHED PROPERTY AND UTILITIES 5.1. Office and equipment. The Governm ent will furnish office space at the Fort Hamilton Chapel with appropriate office furnishing and equipment for the support of the contractor only in the actual performance of products and services. Said office is not permanent or dedicated solely to the u se of the contractor. 5.1.1. Appropriate office furniture and equipment includes a desk, chair, filing cabinet, and, subject to availability, copier services, audio-visual equipment, curriculum, and standard classroom supplies. 5.2. Utilities. The Gove rnment will provide all utilities, such as electrical and water, to perform the services required by the contract, at the site of delivery of the services on federal property. The Government will not make any modifications to utility outlets to accommodat e contractor equipment. 5.3. The Government shall not furnish the contractor with Government living quarters. 6. CONTRACTOR FURNISHED PROPERTY AND UTILITIES 6.1. The contractor shall furnish his/her own transportation to and from the work site(s). 6.2 . The contractor will be prepared to furnish the use of a typewriter and/or word processor, in order to prepare lesson plans, handouts, forms. Statistical data to be submitted as required to the Director of Religious Education (DRE) or to the Installatio n Chaplain. The Government may provide access to such equipment, if available, but is not obligated to do so as a term of this contract. 6.3. The Government will not be liable for loss, damage, theft, or maintenance of personal equipment. 7. CONTRACTOR RESPONSIBILITIES 7.1. The contractor is solely responsible for all tax liabilities. No provisions are implied or provided in this contract for any benefits such as medical care, retirement, or worker's compensation. This is a Non-Personal Service Contr act. 7.2. The contractor shall provide a qualified substitute in the event the contractor cannot meet any obligation of this contract. A qualified substitute is defined as an individual who meets the qualifications outlined in paragraph 2.1 of this cont ract, Competency of Contractor Personnel. Any substitute must meet the approval of the Catholic Chaplain. 7.3. The contractor shall ensure that adult supervision leadership is present not later than 1 0 minutes prior to the start time of the scheduled ac tivities, and remains present until the last participant under the age of 18 has departed the location. 7.4. The contractor shall regard personal information on all applicants, volunteers, and participants as confidential. As required by public laws and military regulations, the contractor will divulge information of illegal activities to the proper authorities. 7.5. The Contractor shall present a neat, well- groomed appearance appropriate for chapel worship services during all times while he/she is performing services under this contract. 8. WORK TO BE ACCOMPLISHED. The work to be performed shall include the following specified requirements. 8.1. Religiou s Education. CCD, Bible Studies, Sunday School, Lunch and Learns and any other religious education program as needed. 8.1.1. The contractor shall plan, coordinate, and conduct weekly CCD/Sunday School programs for all age levels of preschool through adul t acceptable to the Catholic parish, and Installation Chaplain including active supervision of the volunteer religious education staff during the period October - September. 8.1.2. Shall provide monthly inventory, ordering, and distribution services for R E supplies, equipment, and curriculum. Submit monthly requests to Parish Council for support, materials, books, and any other resources necessary for the period October - September. 8.1.3. Shall provide monthly RE communication service including publici ty, monthly calendars and bulletin inserts for the period October thru September. Publicity is defined as hardcopy advertisements, and shall be submitted to the chapel staff for approval. 8.1.4. Shall provide recruiting services to include developing jo b descriptions, interviewing, and placement of teachers and assistants. Placement will ensure that instructional roles fit qualifications, based on subject material, personality, and experience. 8.1.5. Shall provide Advent & Lenten training workshop opp ortunities to assist teachers and assistants. 8.1.6. Shall provide monthly reports to Parish Advisory Council on the status of the CCD and Sunday School enrollment, instructional progress, special liturgical celebrations/events, and requests for resource s for the period November - September. 8.1.7. Shall attend monthly coordinating meetings with the Installation Chaplain, Parish Advisory Council, and volunteers for the period October - September. 8.1.8. Shall prepare annual programs and budget for the entire Catholic / Protestant / Jewish RE program for submission to the Installation Chaplain for inclusion in the Command Master Religious Program for Fiscal Year 2000. 8.2. Vacation Bible School 8.2.1. The contractor shall assist in planning and conduc ting the annual Vacation Bible School program including active supervision of the volunteer religious education staff throughout the event, procurement of supplies and materials, volunteer recruitment and training, and publicity preparation. 8.3. Pastoral Responsibilities. 8.3.1. The contractor shall assist the Chaplain in providing Sacramental preparation for First Reconciliation, First Communion, and Confirmation ; this includes parent sessions, Prayer Services, retreats and receptions. 8.3.2. Shall a ssist the Chaplain in planning and coordinating Liturgies for Advent/Christmas, Lent/Easter, Marian Feast Days, & Baccalaureate Mass and Volunteer Recognition. 8.3.3. Shall provide monthly administration service including organizing and maintaining files , monthly attendance reports, volunteer man hour reports, additional statistical reports as required, and official sacramental record keeping for Confirmation, First Communion, and Reconciliation for the period October - September. Sacramental reports wil l be forwarded to the Archdiocese for Military Services, USA. 8.4. Command Master Religious Program (CMRP). 8.4.1. The contract SOW shall be incorporated into the CMRP by the Installation Chaplain or the Staff Chaplain. 8.5. Chaplain's Fund Council. The contractor shall not serve as a member of the Chapel Tithes and Offerings Fund (herein CTOF) Council but shall be represented by the Installation Chaplain o r Staff Chaplain. The representative shall present the needs of the Catholic/Protestant program. The contractor may attend the meetings of the Fund Council as a visitor and may provide information as required. 9. ADMINISTRATION 9.1. Definition. Admi nistration for the purpose of this contract is the completion of reports, Government forms, and other input that may be required for each contract product service. 9.2. Documentation of Work/Reports. The contractor shall maintain a log of the work perfo rmed under this contract, and shall provide a copy of the log to the Admin Assistant on a monthly basis. 9.2.1. The contractor shall document worked performed on the Invoice or Request for payment. 9.2.2. The contractors payments will be averaged over the length of the contract. 9.3. Request for Supplies and Equipment. The contractor shall provide the Fund Manager with the proper requests for the ordering of supplies and equipment to support the Religious Programs in accordance with the CTOF SOP and the Resource Manager's instructions (for appropriated fund supplies). 9.4. Policies/Regulations. Applicable forms, SOPS, and instructions will be provided to the contractor by the Chaplain Resource Manager. 9.5. Privacy Act. The contractor shall be f amiliar with and insure compliance with provisions of the Privacy Act, Freedom of Information Act, and limitations on Government collection of personal and statistical data on non-affiliated personnel. 9.5.l. Applicable Government regulations concerning these Acts will be made available to the contractor for review by the Staff Chaplain. 10. JUSTIFICATION STATEMENT 10.1. Description: Non-personal services contract for religious education coordinator (REC) to assist Installation Chaplain in implementin g the religious education (RE) for the Fort Hamilton military community per the quality of life standards for religious support. 10.2. What is being procured: Services to coordinate classrooms/setup, curriculum materials and supplies; organize and admini ster a weekly chapel Sunday School, CCD /religious education program and annual Vacation Bible School; Jewish Lunch and Learns and recruit, train and monitor volunteer leaders and teachers for RE and pastoral programs. Coordinate volunteers and environmen t for ordinary and special events. Coordinate parish activities. Assist in preparation of CMRP. 10.3. Why it is needed: Historically, in the major religious faiths RE vies with worship as part of expressing and nurturing spiritual beliefs. The provisio n 52.212-1, Instructions to Offerors-Commercial Item, and 52.212.2, applies to this acquisition.
