Description

The Centers for Disease Control and Prevention (CDC), National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), contemplates awarding a contract to continue gathering evaluation data for its Racial and Ethnic Approaches to Community Health (REACH) 2010 Program. Launched in 1999, REACH 2010 is the cornerstone of CDC’s efforts to eliminate racial and ethnic disparities in health in the following six priority areas: cardiovascular disease, immunizations, breast and cervical cancer screening and management, diabetes, HIV/AIDS, and infant mortality. The racial and ethnic groups targeted by REACH 2010 are African Americans, American Indians, Alaska Natives, Asian Americans, Hispanic Americans, and Pacific Islanders. Award of a Cost-Plus-Fixed-Fee, single contract in accordance with FAR Part 15 is contemplated. The anticipated period of performance is five years from date of award. CDC will consider all responsible sources responding to this Request for Proposals (RFP). All questions/concerns about the RFP shall be submitted via e-mail to BBiltz@cdc.gov. The tentative issue date is February 28, 2005, with the proposal due date of approximately 35 days after posting of the RFP. The complete solicitation package will be available and posted at this site for downloading when issued. No telephone requests of the solicitation will be accepted. No hard copy of the solicitation package will be mailed. The associated NAICS code is 541710 and the small business size standard is $6 million. It is your responsibility to keep up with additional postings regarding the RFP on the Fedbizopps Website.
