Description

The Veterans Integrated Service Network (VISN 6)/Centralized Acquisition Service (CAS) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to purchase two audiometric examination booths and suites as described herein for the Department of Veterans Affairs Medical Center - Durham, 508 Fulton St, Durham, NC 27705. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2(a). The NAICS is 423450 and the small-business size standard is 500 employees. VISN 6/CAS intends to solicit quotations and purchase the following: two (2) IAC 140ACT-sound suites with four (4) 36? doors, two (2) fire rated panels, six (6) windows in doors, four (4) locks, four (4) door closers, and all associated installation & freight. Offeror shall include warranty information as appropriate with offer. Quotations shall be submitted on an all-or-none basis. Partial quotations shall be rejected. Quotations shall be valid 60 days from offer due date. Prices are F.O.B. Destination. All responses shall be for the following specified manufacture/brand name or equal: Industrial Acoustics Company (IAC 140ACT). Equal offers shall include: 1) descriptive literature and product number for time offered; 2) Separate unit cost of each item; 3) evidence of past performance of contracts of similar type, scope size and complexity that are ongoing or completed within the past three years, references with points of contacts must be identified, 4) delivery time. Offerors must be able complete installation no later than February 28, 2006 to be considered for award. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offerors-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items; 52.211-6 Brand Name or Equal; 852.211-77 Brand Name or Equal; 52.247-34 F.O.B. Destination; 852.270-1 Representatives of Contracting Officers; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items including clauses incorporated by reference under section b (5) (14-20) (23) (26) (32). In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at http://orca.bpn.gov/ in accordance with FAR 4.1201(a). This is a best-value procurement. Award will be made to the offeror whose offer conforming to the solicitation is determined to be most advantageous to the Government, price and other factors considered. The Government intends to evaluate offers and award without discussion. Offeror shall submit all information with offer as required in 52.212-1 and 852.219-70, Veteran Owned Small Business Certification. A pre-bid site visit will be held at the VAMC-Durham on November 30, 2005 at 9:00 am local time. Offerors are urged and expected to inspect the site where the work will be performed. Offerors attending the pre-bid site visit shall contact Ms. Helen Warren at (919) 286-6920 no later than November 29, 2005 at 3:00 pm local time. Quotations must be received by December 13, 2005, 2:30 pm local time. Quotations shall be submitted in hard copy to: Robert Betts (90/C), VAMC - Hampton, 100 Emancipation Dr, Hampton VA 23667.
