Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote is W9124B-06-Q-0002 issued as request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Ac quisition Circular 2005-05 dated 27 July 05. The associated NAICS code is 813110. DIRECTOR, CHAPEL YOUTH ACTIVITIES for the YOUTH RELIGIOUS ACTIVITIES SERVICES 1. General. Youth of the Chapel (YOC) Program falls under Installation Chaplains Program. The mission of the Chaplains program is to provide for religious support needs, to include religiously based youth study and outreach programs, of all assigned soldiers, civilians, and family members (see AR 165-1). The primary goal of the YOC program is to provide basic religious understanding of life events through fellowship activities, youth meetings and events, Bible studies, retreats, camps and mentoring programs involving youth of assigned installation personnel. The specific and professional Yout h Religious Activities services required by this contract supplement the basic religious education YOC programs conducted by the chaplain. Persons providing Youth Religious Activities services are professionally trained specialists in youth activities. T he persons providing the services of this contract either as an individual non personal services contractors or as an employee of a vendor contracting to provide such services are neither members of the Installation Chaplains Office Staff nor employees of the Government. 2. Scope of Work. The purpose of the Youth Religious Activities services non personal services contract is to provide appropriate opportunities for middle school (grades 6-8) and senior high school (grades 9-12) students to develop a plur alistic, spiritually or religiously based framework for daily living.Youth Religious Activities services are specifically non-denominational in scope, applied within a pluralistic military community environment, and must accommodate the free exercise needs of all participants. The need for specialized Youth Religious Activities is outlined in AR 165-1. 3. Location of Youth Activities and or Events. Arrangements for facility use after normal duty hours or at times other than when weekly religious services are held will be the sole responsibility of the contractor. 4. Hours. Generally, Youth Religious Activity meetings and events will be provided during hours that encourage maximum youth participation. These would normally be evening and weekend hours. Th e contractor may be required to provide services on legal federal, state, and local holidays. 5. Contractor shall provide the following specific tasks: ITEM DESCRIPTION AND QUANTITY FOR FIRM FIXED PRICE (FFP) CLIN0001:Youth Group Meetings. Plan, coordinate and conduct group meetings, normally of one hour in duration and accommodating groups of three or more, for middle and senior groups focusing on spiritual, ethical, and moral lifestyle. Provide for Catholic and Collective Protestant requirements. Apply a needs assessment survey to scope the meetings, programs, and activities to meet the needs of the target demographic population and attendees; Qty 72 Each. CLIN0002: Special Events. Plan, coordinate and conduct special events, which may consist of communi ty service projects and other similar activities, monthly for each middle and senior high group, which focus on team building, community responsibility, spiritually based lifestyle, and faith sharing. Special events will normally consist of activities inv olving ten or more youth within the community and be of at least one 8 hour workday in duration. A special event such as a community service project may be cond ucted over several days. Administer a needs assessment survey to scope or determine the activities to meet the needs of the target demographic and the attendees. Submit After Action Reports to the Installation Chapel POC within one week of completion of t he special events; Qty 18 Each. CLIN0003: 5-Day Camps. Plan, coordinate and conduct one 5-day camp for each middle and senior high group. Prepare and submit a program plan that includes camp dates and locations, schedule of activities and study topics, ob jectives, method of evaluating program, funding requirements, facility needs, and volunteer requirements. Camp shall supplement the Installation Youth of the Chapel program and shall not include camps or retreats sponsored by any other institution or orga nization. Submit After Action Reports to the Installation Chapel POC within one week of completion of the camps; Qty 2 Each. CLIN0004: Retreats and or Lock-Ins. Plan, coordinate and conduct retreats and/or one over-night lock-in for middle and senior hi gh groups. Provide Installation Chapel POC with a program plan to include location, time, date of event, objectives, method of evaluating program and resource requirements no later than 30 days prior to the event. Submit After Action Reports to the Insta llation Chapel POC within one week of completion of the retreats or lock-ins; Qty 0 Each. CLIN0005: Community Service Project. Develop, write, and implement a one five-day community service project program plan that includes the project name, dates, and objectives; and methods of evaluating program in accomplishing objectives; required funding and types of funding to fulfill program. Submit After Action Reports to the Installation Chapel POC within two weeks of completion of the community service project s; Qty 0 Each. CLIN0006: Volunteer Management. Train volunteers that support and participate in the Youth Religious Activities programs and events as required. Maintain volunteer utilization files to include volunteers name, recruitment date, utilizat ion, training, and hours volunteered. Maintain records of all youth activities and or events to include number of volunteers utilized and number of persons attending each event; Qty 4.5 Each. CLIN0007: Meetings. Meet with the Installation Chapel POC on ce a month (one hour) to discuss quality of the delivery of the services of this contract, program statistics, and progress. Provide a monthly statistical report on the number and types of youth events, attendance at each event, completed events, list of events planned for the next 90 days, publicity plan for programs, number and type of training events conducted and training attendance; Qty 4.5 Each. CLIN0008: Reports. Provide quarterly reports on the number and type of youth events, attendance at each event, completed events, list of events planned for next 90 days, publicity plan for programs, number and type of training events conducted, and training attendance; Qty 3 Each. CLIN1001: Youth Group Meetings. Provide a monthly statistical report on the number and types of youth events, attendance at each event, completed events, list of events planned for the next 90 days, publicity plan for programs, number and type of training events conducted and training attendance; Qty 6 Each. CLIN2008 Reports. Provide quarterly reports on the number and type of youth events, attendance at each event, completed events, list of events planned for next 90 days, publicity plan for programs, number and type of training events conducted, and training attendance; Qty 4 Each. 6. Contractor shall accomplish the following implied tasks in conjunction with youth meetings, special events, retreats, and camps. The following requirements are common expectations for all deliverable end-services under this contract. a. Market and advertise the programs, events, and activities to include camera ready copy to the Installation Chaplain's office for advertising and promoting youth activities by use of chapel bulletins, the post bulletin, bulletin boards, flyers, the community newspaper, and any other appropriate media. b. In accordance with AR 165-1 and directives from the FORSCOM Chaplains Offic e, the Contractor shall be responsible for maintaining records and files incident to the services provided in this contract. Files must be maintained in a secured area (an office area that can be locked) and are the property of the U.S. Government. c. Be present in the community school and other sports facilities using participation in various sports activities as an opportunity for acquaintance, meaningful verb al interaction, and to inform the students of club, chapel and other youth activities in the community. d. Conduct a ministry of presence, making contacts with youth in the community by attending school, sport, social, and community activities where youth are expected to be present. 7. Qualifications. Individuals providing youth ministry or employees of a contractor providing such services must have training in Youth Religious Activities as evidenced by: a. A Bachelors Degree in the area of youth minis try, or any other ministry field from an accredited university or Any degree from an accredited university with two years of work experience (paid or volunteer) as a Youth Minister, Bible School teacher, Sunday School Superintendent, Director of Religious Education, Christian School teacher or related field. b. Contractor and or the contractor's employees shall possess the ability to communicate effectively with chaplains, volunteers, and government representatives in English orally and in written format ( i.e. advertising and marketing, instructions, invoices, etc.). Ability to speak English clearly and excellent comprehension is essential. c. Contractor and or contractors employees shall have the ability to interact, support, and encourage effectively wi th individuals from different religious groups through religious pluralism concepts. d. Contractor should possess the knowledge and able to articulate the application of the 1st Amendment of the Constitution of the United States of American pertaining to t he freedom of religious expression in a military environment. e. Background check: According to DA Circular 690-95, AR 215-3 and AR 608-10, before any contractor or any contractor employees begin classes, all persons working in direct contact with child ren under the age of 18 must submit to a background check to ensure such individuals have had no allegations of improper behavior involving child abuse or drug use. 9. Disclosure of Information. In the course of providing youth activities and events, th e contractor may come in contact with individuals seeking the assistance of chaplains. The very fact that individuals are seeking assistance with family and or personnel issues is at minimum sensitive information. Complete confidentiality is paramount. The sharing of information about individuals or families seeking counseling assistance from chaplains with persons other than the POC for this contract or the chaplain involved is strictly prohibited. Disclosure of sensitive information regarding chaplain counseling appointments and individuals involved may result in immediate termination of the contract. 10. Support: a. Contractor shall be authorized, on a space available basis, use of government telephone (with commercial access) for local and long dist ance calls to maintain contact with volunteers in the program. No reimbursement for calls placed in the conduct of business is authorized. d. The Government will provide to the contractor all curriculum materials for youth Bible studies, chairs, tables, blackboards, video set-ups, and overhead projectors for all youth events, classes, or activities. Materials such as craft supplies, paper, markers, flip charts, and pens will be supplied as available. e. The contractor or employees of the contractor may b e authorized specific and limited logistical support. 11. Contractor Furnished Property. Contractor shall provide his/her own transportation and all necessary equipment to fulfill this contract except as stated elsewhere in this contract. 12. Physical Security. The Contractor shall establish and implement methods to ensure the physical security of all U.S. Government property and facilities under their control IAW facility, and Installation SOPs. Locking of external doors and windows is necessary whe n leaving the facility. Internal doors will be locked to protect Government property. Access to office areas, especially those with telephones and automation equipment must be closely monitored and limited. 13. Safety. The Contractor shall be aware of local safety requirements and ensure that all reasonable efforts are made to protect all persons from harm and the U.S. Government property from damage. 14. Restrictions. Government property made available under this contract is only for use in the performance of services described in this contract. 15. Disclaimer. The government will not be responsible in any way for damage to the contractors supplies, materials, equipment, or to personal property as a result of fire, theft, accident, natural disaster, or other events. 16. Liability. For the purpose of this contract, the Contractor is considered an independent contractor and not as an agent of the U.S . Government. During performance of services, the Contractor shall be liable and will indemnify and hold harmless the Government, its agents and employees against all actions or claims for damages to persons or property, including death arising from fault , negligence, wrongful act, or wrongful omission of the Contractor, his agents or employees utilized in the provision of services. 17. Compensation. The contractor will be paid upon satisfactory completion of the services contained in by the SOW and upo n presentation of invoice IAW terms of this contract. The Contractor is authorized partial payments on a monthly basis. By intent, this contract is a non-personal services contract and does not create an employer-employee relationship. There are no prov isions for lodging or employee benefits of medical care, retirement, or workers compensation. The contractor is responsible for his/her own tax liabilities. Payments made under this contract are not subject to Federal Income Tax withholding. 18. Subcon tracting: Contractor may provide a qualified substitute for a period of time if approved by the Contracting Officer. However, in such case, the substitute shall have no claim whatsoever against the Government for services rendered and the price to be pai d the substitute shall be a matter entirely between the Contractor and the substitute. Additionally, any substitutes must be approved by the Government. Requested Period of Performance: 1 JAN 2005 to 30 SEPT-2008.
