Potential sources sought to participate in the regional standardization of Full Product Line of Exam Gloves (Vinyl, Latex, Synthetic) The review process leading to a selection of vendor(s) upon which to standardize will take in to consideration, but not be limited to, the following technical and clinical criteria: TECHNICAL CRITERIA 1 A Distribution and Pricing Agreement (DAPA) with Defense Supply Center Philadelphia is a requirement for this standardization initiative. Provide your DAPA # __________________________ and DAPA Expiration Date: ______________________ 2 Under what professional standard(s) is the manufacture and distribution of your Exam Gloves product line and services guided? (Ex: ASTM; FDA; OSHA; ADA) 3 What certification(s) does your company maintain appropriate to the Exam Glove products and service industry? 4 Indicate whether the source(s) of your Exam Glove products and services is domestic or foreign. 5 Indicate the location of your production/processing/distribution facility appropriate for the Northeast Region Medical Treatment Facilities and Clinics. 6 Under what brand name(s) does your company produce and distribute Exam Glove products and services? 7 Which sterilization/sanitizing method(s) is used by your company in the production and distribution of your Exam Gloves? 8 Describe how your company regulates the manufacture, storage/stock rotation, shipping and the delivery of Exam Gloves to assure the integrity and cleanliness of your product line? 9 Are the Lot Number, manufacturing date/location, expiration date, pre-cautionary content labeling easily identifiable on the Exam Gloves packaging 10 How does your company clearly label the latex content of your Exam Gloves 11 What is the failure rate for the Exam Gloves? 12 Describe the packaging options for your Exam Gloves. 13 How does your company clearly label the powder/powder residue content of your Exam Gloves 14 Provide documented evidence of skin sensitivity testing with your Exam Glove products. 15 Provide documented evidence of Exam Glove compatibility with alcohol-based hand sanitizers/surgical hand scrubs and associated hand lotions. 16 Provide documented evidence of additives used during and/or after manufacture of your Exam Gloves. 17 Describe your policy and actions when an Exam Gloves product/product line will no longer available or will be substituted by another product 18 Describe your clinical representative availability. Indicate whether representative assigned by facility or by geographical area. 19 Is the assigned clinical representative for each facility dedicated solely to the Exam Gloves product line or is responsible for other non-Exam Gloves product lines? 20 Describe the available periodic program/service updates and what level of consultant is providing the updates. 21 Describe your start up or implementation program. 22 Describe your ongoing clinical/educational support program(s). Include such capabilities as sales/utilization reports, product change reports, CEU offerings, etc. 23 Distribution through our Regional Prime Vendor (currently Owens-Minor) is required for this standardization initiative. Describe your business relationship with our current Regional Prime Vendor (Owens-Minor) (requirement for potential selection for standardization for Exam Gloves). 24 Describe your capabilities to meet the Northeast Regional volume requirements through the current Prime Vendor (Owens-Minor). Include solutions to prevent back-order from your company stock, standard vendors, and customer choice/regional standardized vendors. 25 Describe your capabilities for responding to heightened inventory and supply needs for operational (wartime) casualties on short notice. 26 Describe your no fee/no cost value-add services and programs. 27 Provide a detailed history of Exam Gloves recalls experienced by your company. Include reason for recall, action to resolve, and total time period to resolution. CLINICAL CRITERIA 1 Full product line: latex, vinyl, synthetic 2 Full line of sizes: extra small to extra large 3 Size easily identifiable by color coding, package labeling, etc. 4 Ambidextrous 5 Ease of dispensing from packaging container 6 Well-fitting 7 Flexible 8 Easy to don 9 Beaded Cuff 10 No unpleasant odor 11 Textured grip sufficient to easily grasp small and/or wet objects 12 Tactile sensitivity sufficient for to easily grasp small objects 13 Tape/label adhesives remove easily 14 Will not easily rip or tear 15 Not irritating to the skin. 16 Comfortable to wear for long periods Tri-Service Northeast Region, an integrated military health care delivery network comprised of Army, Air Force, and Navy Medical Treatment Facilities (MTF), is considering standardizing and establishing a Regional Incentive Agreement(s) {RIA(s)} for a Full Product Line of Exam Gloves. This is not a request for bids but an invitation to participate in a product line review process that will result in a selection of vendor(s) to provide this product line to MTFs within the Northeast Region. Vendors must pro vide full product line literature and available documentation of research and development in the design and performance of this product line. Vendors must also provide product line samples and knowledgeable and appropriate representatives as needed for po ssible vendor demonstrations and possible on-site clinical evaluations/trials. All expenses related to literature reproduction and dissemination; product line demonstrations; marketing; clinical evaluations/trials; and appropriate in-service education rel ated to the above throughout this standardization initiative must be at no charge to the government. Vendors, suppliers and manufacturers must be holders of a Distribution And Pricing Agreement (DAPA) with the Defense Supply Center Philadelphia (DSCP), distribute products and associated product line supplies/consumables through the DSCP Prime Vendor (PV) program, and satisfy the technical and clinical criteria for product line design, performance and vendor support programs as put forth in this standardization initiative. Vendors, suppliers, and manufacturers must hold a DAPA to be eligible to participate in this standardization initiative.
