Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will NOT be issued W91QFT05-06-T-0002 is being issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2 001-23. The NAICS code applicable to this acquisition is 484210. Statement of work The contractor shall furnish all labor, materials, transportation and equipment required to move Pharmacy Services in its entirety from the first floor to the second floor of Building 8130, Kenner Army Health Clinic. The move must be completed in no more than 4 days, to include a weekend (Saturday and Sunday).Tentative start date will be Friday, 13 January 2006, with a completion date of Monday, 16 January 2006 (Federal Hol iday). The government reserves the right to delay the beginning date by providing the contractor seven days advance notice. No change will require the Contractor to initiate early action. Contractor will be required to disassemble, move and reassemble all shelving, modular units, furniture and equipment located in the Pharmacy Services. Access to designated areas will be available starting 9 Jan 06 to allow the contractor to disassemble, move and reassemble items unneeded by the pharmacy for daily ope rations. Contractor will be required to provide temporary shelving for government personnel to utilize while the shelving systems are being moved. Temporary shelving will be used by pharmacy staff to hold pharmaceutical stock for access and dispensing to customers during the moving process. Insurance Coverage Contractor will provide sufficient insurance coverage to repair and/or replace any office equipment or furniture lost, damaged, or made non-usable for its intended purpose by actions taken by the c ontractor or his/her representative. Additionally, insurance coverage shall be provided by the contractor to cover any repairs to government buildings because of damage by the contractor or his/her representative.. Conduct of Personnel: All personnel emp loyed by the contractor in the performance of this contract or any representative of the contractor entering the government complex shall conform to all security regulations that may be in effect during the contract period and shall be subject to such chec ks as may be deemed necessary to assure that no violations occur. No employee shall be permitted on the Government complex when such check reveals that his or her presence would be detrimental to the security of the installation or accomplishment of the w ork. The Government may bar employees for just cause. This does not relieve the Contractor from his obligations under the contract. Removal of Contractor's Employees The Contractor shall employ, to perform work for the Government, only experienced, res ponsible and capable individuals in the performance of the work as defined herein. The Contracting Officer may remove the Contractor who endangers persons or property, or whose conduct is prejudicial too the best interest of the Government. The provision s at 52.212-1, Instructions to Offerors Commercial, applies to this requisition. Offerors are requested to include a completed copy of the provision at 52.212 3 Offeror Representations and Certifications Commercial Items. The clause at 52.212 4, Contract Terms and Conditions Commercial Items, applies to this acquisition; addenda to the clause are not included.
