Description
Solicitation for the possible replacement of hospital plastics for TRICARE Puget Sound and Alaska. These products include but are not limited to a complete product line in a variety of sizes and colors. Those companies interested in a possible Regi onal Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Puget Sound and Alaska must respond to the following questions: 1. Does your company carry a full product line of hospital plastics (pitchers, emesis basins, bed pans, drinking cups, specimen containers and collectors, etc) to include different sizes and colors. If so, provide a list. 2. What are the standards by which your products are manufactured? Comply with NIOSH, OSHA and industry standards? 3. Does your company have a distribution agreement with Cardinal Healthcare? 4. Does your company currently possess, or is in the process of obtaining, a Distribution and Pricing Agreement (DAPA) through Defense Supply Center, Philadelphia? 5. Does your company have the ability to service to the entire TRICARE Puget Sound and Alaska Region (WA and Alaska)? Please provide your companys distribution network. 6. Can your company provide customer support (1-800-numbers and product representatives) for each account? 7. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will product in-service and education be available upon request after product implementation throughout the length of a possible agreement? 8. Describe your companys return goods policy. 9. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Puget Sound and Alaska. 10. Does your company provide volume discounts? 11. Provide a history of back orders and/or recalls for this product group with dates, duration, cause, how information was communicated to the customer, interim measures and final resolution. 12. Does your company distribute or manufacture your product line? If you are a distributor, what brand(s) do you distribute? Product Criteria for Hospital Plastics: 1. Does your company carry latex-free products? 2. What material(s) are your containers made of? (a) Is this material durable, sturdy and non-collapsible? 3. Is non-toxic, color fast dyes used on your containers? 4. Do your products have smooth edges? 5. Is the use and function of your products easily identifiable? 6. Are the numbering and characters used on your products readable? 7. Do your products have an accessible area for easy application of the patient ID label? 8.
