Description
The Centers for Disease and Control Prevention (CDC), National Center For Chronic Disease Prevention and Health Promotion (NCCDPHP), Health Care and Aging Studies Branch (HCAS), Division of Adult and Community Health (DACH), is planning, coordinating, and implementing a CDC public health program on aging. To that end, the CDC is in need of enhanced expertise in ?healthy aging? issues to include gerontology, aging research, and the cross-over of aging services and public health. The CDC needs an external advisor/consultant who can bridge the gap between aging services and public health, and has knowledge in all areas of aging, particularly, the Administration on Aging (AoA), the Older Americans Act (OAA), and OAA supported National aging services network, and an array of national aging organizations to provide on demand expertise. Overarching tasks will involve, 1) developing a public health & aging agenda, 2) developing partnerships and 3) collaborative activities with external organizations, especially within the national aging network, and 4) assisting with the continuing implementation of and enhancement of CDC?s National Program to Promote Healthy Aging and the ?Healthy Aging program?s Alzheimers Cognitive vitality initiative. The contractor shall serve as a subject matter expert (SME), provide technical assistance and consultation to HCAS Aging program staff, and perform activities necessary to provide the required expertise and services involved in 1) advising and making recommendations for continuing, shaping, and improving CDC?s National Program to Promote Healthy Aging, 2) fostering and expanding links with experts in aging research and ?Healthy Aging? service providers, 3) developing and refining partnerships with key external organizations particularly from the national aging network 4) advising and participating in activities that help shape the CDC public health strategic direction for aging and 5) advising and making recommendations for the development, shaping, and improvement of the ?Healthy Aging? program?s Alzheimer Disease cognitive vitality initiative. The contractor shall provide all labor, materials, and supplies, to perform professional services necessary to develop partnerships with key external organizations, provide recommendations for the implementation, refinement, and improvement of CDC?s National Program to Promote Healthy Aging and the ?Healthy Aging? program?s Alzheimer?s Disease cognitive vitality initiative. The period of performance shall be from the effective date of the award through September 30, 2006. Government furnished property shall include CDC, Division and Branch organizational charts, Previous Aging Advisory meeting documents, Names and contact information for potential external partners, and when available, copies of the operational and strategic planning documents.
