- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91248-06-Q-0020 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acqu isition Circular 2001-20. The NAICS Code is 621340 and the small business size standard is $6,500,000. Performance period is for one (1) 12-Month Period. Performance is required to start on or about 01 April 2006 through 31 March 2007. A teacher/trainer must have completed courses in a (a) CPR, (b) basic athletic training, (c) advanced athletic training, (d) must have or be working toweard his/her at hletic training certification. Must be in attendance at all football practices and games (including daycamp). Must be at all daily team practices. Must attend all other home sporting events (soccer, basketball, volleyball, softball, etc.) as assigned by the Athletic Director or Principal. Give basic instruction to each sports staff and athletics on prevention and care of injuries. Available for taping, wrapping and bracing before each practice and sporting event. Train and supervise student trainers. Develop and supervise rahabilitation for all athletics (working with the physician in charge of that athletics care). Decide when an injured athlete may return to practice/game. Maintains regular training room hours, before school, during lunch, and af ter school. Is responsible for the maintenance, organization and requisitioning of all training room supplies. Supplies parents with information on nutrition, care for injuries, training room procedures, and a physical if needed. Develop and implement a n Emergency Medical Plan for severe injuries.
