- This is a combined synopsis and solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, reference number is RFP 246-06-02528. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-07. This acquisition is an Open Market, Unrestricted procurement. The associated North American Industry Classification System (NAICS) for this procurement is 621512 and the Standard Size is $11.5 million. The contractor shall provide all equipment, facility, professional and technical personnel services to provide routine x-ray films for eligible veteran beneficiaries as referred by the Jacksonville Community - Based Outpatient Clinic (CBOC) a part of VA Medical Center, Fayetteville, N.C. Interested offerors place of business must be located within 15 miles of the Jacksonville CBOC which is located at 1021 Hargett Street, Jacksonville, NC 28540. Contract will be effective for a period of one year effective April 1, 2006 through March 31, 2007 with four (4) one year options to extend, subject to the availability of funds. The provisions at FAR 52.212-1, Instructions to Offerors--Commercial Items, apply to this solicitation. The provisions at FAR 52.212-2, Evaluation-- Commercial Items, apply to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforms to the solicitation, and is the lowest priced technically acceptable offer. The clause at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation and the offerors are requested to complete registration of their Representations and Certifications On-line at http://orca.bpn.gov/publicsearch.aspx or provide manually completed Representations and Certifications with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation. All interested parties must be registered in the Central Contractor Registration (CCR) database at www.ccr.gov.
