- (Sole Source-urgent) STATEMENT OF WORK 1. Contractor shall provide all labor, materials, parts (including software updates, components, equipment, transportation, and supervision necessary to perform inspections, and to complete preventive and corrective maintenance, such as calibrate, adjust and test Smith and Nephew-owned, government leased equipment. 2. Contractor agrees that all services will be performed on-site by trained and certified technicians qualified to work on all Smith and Nephew systems. No piece of equipment will be removed from Naval Hospital, Camp Pendleton without written consent from the Head of Biomedical Repair or the Plant Account Clerk. 3. Contractor shall perform four (4) pre4eventive maintenance inspections during the base and option years. The inspections are to be conducted in December, March, June and September. Times and dates shall be coordinated between the contractor and the Head of Biomedical Repair a minimum of ten (10) days before the PM is due. a. Preventive maintenance inspections covers such items, but is not limited to lubrication, cleaning, mechanical and electrical adjustments, functional tests, and verification of electrical calibrations, performed to the original equipment manufacturer (OEM) specifications. b. Preventive maintenance inspections shall include the replacement of all worn and/or failed components, including worn or frayed electrical wires or power cords. If statistical data on equipment indicates failure point is approaching for any component, said components shall be repaired or replaced in advanced to prevent system downtime or failure. 4. The Contractor will furnish and install all required parts and materials including cameras and scopes except consumables, used during all preventive and corrective maintenance at no additional cost to the government. Replacement parts must be new, certified, and in compliance with manufacturer?s specifications. Contractor shall use only OEM-approved replacement parts and lubricants. 5. There shall be unlimited emergency services during normal working hours, 0800 a.m. to 1700 p.m. Monday through Friday. Holidays and overtime are excluded. Contractor guarantees a response time within four (4) hours by phone, with a technician on-site within twenty-four (24) hours from receipt of a maintenance request. Overtime work (after 1700, weekends and holidays) is not authorized under the terms of this contract. If overtime is necessary, it will be covered under a separate purchase order and shall be at the prevailing company rate. Contractor shall provide their overtime rates. 6. Contractor will make any engineering changes or updates to equipment as required by the manufacturer or regulatory agencies for operator and patient safety or increased equipment reliability at no cost to the government. 7. If the repair requires longer than twelve (12) hours, the contractor shall provide a replacement at no cost to the government within 24 hours. Prior to this service or any service to be performed, the representative will report to Biomedical Repair Section, room 104, Bldg H-135, Naval Hospital Camp Pendleton, CA. A copy of the completed service report shall be furnished to Biomedical Repair Dept. The service report shall include the following but not be limited to the following information: i. Model and Serial Number of repaired or replaced equipment ii. PM control or plant account number of item repaired or replaced. iii. Labor hours expended. iv. List of parts used and costs. v. A brief summary of repairs, inspections, and maintenance. Printed name and signature of the designated department representative.
