JOINT ANALYSIS CENTER/RAF MOLESWORTH SANITARY DISPOSAL AND LAVATORY DEEP CLEANING SERVICES SITE VISIT The site visit for this requirement is on Fri., 24 March 2006 at 1300 and will take place at the Joint Analysis Center, RAF Molesworth. If you have not already done so, please e-mail your proposed attendees to richard.early@alconbury.af.mil by the March 22. You will need to provide full name, date of birth and full address. You will need to bring either a current passport or photo I.D. (drivers license) with you. To sign your vehicle on base you will need to produce a current driving license and current road tax disc. All attendees will need to be at RAF Molesworth Main Gate , no later than 1245:45 on the day of the site visit. You will be welcomed by a representative (TSgt. Dave Ouellette) from the Contracting Office and then proceed to the the Joint Analysis Cener where the site visit will start at 1300. Due to security restrictions, your representatives will not be able to physically inspect all of the facilities. Access to secured facilities will only be granted to the successful party after contract award. The facility manager will escort your representatives to several building where security is somewhat less stringent so that you can see what the typical lavatories and other areas look like. No questions will be answered on the day. EARLY Contracing Officer QUALITY ASSURANCE SURVEILLANCE PLAN FOR Joint Analysis Center/RAF Molesworth Custodial Services JAC/RAF Molesworth 1 Apr 06 QUALITY ASSURANCE SURVEILLANCE PLAN FOR Custodial Services SERVICE: Basic Latrine Deep Cleaning Services and Sanitary Disposal Services SURVEILLANCE: The Quality Assurance Personnel (QAP) is the POC for complaints received from Joint Analysis Center personnel. QAP will pass customer complaints to the contractor's head of deep cleaning/ sanitary disposal services on site manager for correction. STANDARD: The contractor shall perform all work required by the Performance-Based Work Statement (PWS) in a satisfactory manner. Recurring defects are not permitted for any of the above service items. Complaints by personnel or customers should be corrected immediately. Any repeat of unacceptable performance will be reported by the QAP to 423 ABG Contracting Office for appropriate action in accordance with FAR 52.212.4, Contract Terms and Conditions-Commercial Items. PROCEDURES: Any personnel or customers who notice, receive or experience substandard services will give it to the QAP for correction. If the infraction is not corrected, incident(s) will be reported immediately to the Head of laundry services/on site manager, then to the QAP. QAP will conduct his/her own investigation/observation before notifying the servicing contracting office when the problem is not corrected or if any repeat of unacceptable performance. The QAP will report the discrepancies to the Contracting office by completing a receiving report. The surveillance method utilized for this contract shall be by customer complaint and by random QAP inspections. Customer complaint training will be the responsibility of the Functional Commander/Functional Director and carried out by the QAP. Training will be no less than quarterly and will be carried out via e-mail to all Joint Analysis Center. Copies of the Customer Complaint Training notices/e-mails will be printed and retained in QAP records.
