Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ) for the implementation of a blanket purchase agreement (BPA) Submit written quotes (oral quotes will not be accepted). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-08 (iv) The NAICS Code for this synopsis/solicitation is 611692. (v) This combined synopsis and solicitation is for the purchase of a motorcycle safety training course to be administered on Malmstrom Air Force Base, MT. (vi) CLIN 0001: The Government intends to award one contract line item number (CLIN 0001) for a three (03) agreement for a Motorcycle Safety Training Course. 1.1. Summary of work The contractor, shall provide all labor (instructors), equipment (motorcycles & helmets), and training material (work books, state regulations, videos and testing material) to properly train, evaluate and test enrolled motorcycle students. All classroom services will be accomplished at the Health and Wellness Center Building 1145, Rm. 101 or Wing Headquarters Building 500 Rm. 153P on Malmstrom AFB and the riding portion and evaluation portion of the training will be conducted on the old taxi way by Building 250. 1.2. Operational Requirements and Specific Tasks a. Contractors will provide proof of insurance coverage. This agreement will be awarded only to a responsible contractor with the required experience, training and certification so that all students meet the requirements set forth in Department of Defense Instruction (DoDI) 6055.4 and Air Force Instruction (AFI) 91-207, Message: ?MOTORCYCLE RIDER EDUCATION TRAINING REQUIREMENTS? dated 9 Nov 1999 and Montana State Law. In accordance with DoDI 6055.4 and AFI 91-207, instructors must be trained and certified by the Motorcycle Safety Foundation. A valid motorcycle safety foundation card is the the sole certification accepted by the standards set forth in the aformentioned guidance. b. Wing Safety staff shall, weekly, provide the contractor with all the students on the training waiting list, and any additional students that request a class slot after the contractor is provided said list. The contractor shall complete all the scheduling for each class, and then provide Wing Safety with a class roster prior to the beginning of each class. The contractor shall schedule each student provided to them not to exceed 24 total students (6, 12, or 18 out of 24 reserved for military personnel contingent on the number of instructors available agreed upon by 341 SW Safety Office). The contractor shall be responsible for all scheduling and no-shows. c. The contractor shall teach the Basic RiderCourse (BRC) at Malmstrom AFB from April through October. Classes will not be conducted on 3-day federal holiday weekends. Exception: A significant world-wide event that would preclude contractor access to the installation and availability of facilities. Also includes instructor availability due to illness and lack of student participation as agreed upon by the 341 SW Safety Office. 1.3. Quotes should be made on a per student service basis. In addition, the contractor should state whether they are able to provide all of the listed services. 1.4. Services include: a. Basic RiderCourse (BRC) b. Registration 1. Current Roster of Students per Class c. Invoicing 1. Invoice with Roster Following Each Course d. Record Keeping 1. Database of Student Information Beginning in 1996 **ALL contract employees (instructors) must be able to qualify for base driving privileges through the Malmstrom AFB 341st Security Forces Squadron. Once this is completed, they will be added to an entrance listing for Malmstrom AFB and listed training facilities listed above. 1.5. Extent of Obligation The government is obligated only to the extent of authorized calls placed against the Blanket Purchase Agreement. 1.6. Call/Pricing Limitation A call is defined as a request for services for two days of training on the Basic RiderCourse (BRC). The Government will place calls one month in advance for the training course. Authorized personnel will generally make calls against this agreement orally. Authorized personnel will be identified in writing by the Contracting Officer and provided to the contractor upon award. Calls placed by individuals other than those authorized shall not obligate the Government. No individual call under this agreement shall exceed $24,000. 1.7. Notice of Individuals Authorized to Enroll Students The 341st Space Wing Safety Office shall furnish a list of names of individuals authorized to place calls under this agreement, identified by the using activity, to the supplier. 1.8. Calls Calls against this Blanket Purchase Agreement shall generally be made orally. Call numbers will run consecutively throughout the life of the BPA. The contractor will be called as to which location they need to dispatch on a weekly or as needed basis 1.9. Price List A list establishing the prices for required services will be provided to the Contracting Officer by the Contractor on an annual basis or when requested by the Contracting Officer. 1.10. Payments Payment will be made with the Government Purchase Card (GPC ? VISA Card) 1.11. General Information a. The contractor shall provide: traffic cones/lane marking devices required for the riding range; a sufficient number of motorcycles for use by students and the instructors for demonstration purposes; and required safety equipment for instructors, same as required for students. b. The contractor shall provide all insurance, fuel, and maintenance costs for contractor-furnished motorcycles, as well as any damage costs incurred during training. c. The contractor shall provide the most current available approved course curriculum materials including: all student handout materials, study guides, and test materials for the course instruction. d. The contractor shall provide a Department of Transportation (DOT) approved helmet for students in the BRC. e. The contractor shall be responsible for any injuries sustained by contractor personnel during training. f. The contractor shall be responsible for a student roster for each completed course furnished to the 341 SW Ground Safety Office (SEG). The roster shall annotate full name, date of birth, mailing address, home telephone number and/or work telephone number of students successfully completing the course, and include names of students who failed due to academics or riding skills, and those scheduled for the course that failed to attend, or to stay for the duration of the course. g. The contractor shall provide completion cards for all students upon successful completion of the course. The completion card is valid for obtaining a skill test waiver at any Montana driver licensing station for one calender year from date of issue. h. Students shall provide personal protective equipment consisting of the following: --Full fingered gloves designed for use on motorcycles.(mittens not recommended) --Long trousers. --Long sleeved shirt or jacket. --Sturdy footwear: Leather boots or over-the-ankle shoes. --Proper eye protection (i.e., impact or shatter resistant clear goggles, or full-face shield). 1.12. Base Access/Force Protection a. Government Property. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work shift, all government facilities, equipment, and materials shall be secured. b. All contract employees (instructors) must be able to qualify for base driving privileges through the Malmstrom AFB 341st Security Forces Squadron. Once this is completed, they will be added to an entrance listing for Malmstrom AFB and listed training facilities listed above. c. Restricted Access Entry. Base Access. The contractor shall obtain personal identification photo passes for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto Malmstrom AFB from Security Forces Pass and Registration, for the duration of the agreement. Vehicle registration, proof of insurance, and a valid driver?s license must be presented for all vehicles to be registered. During Force Protection Condition (FPCON) of Normal and Alpha, personnel without base issued identification shall be sponsored onto the installation. During FPCON Bravo, Charlie and Delta, personnel without base issued identification shall be physically escorted onto the installation. The contractor shall control access to all government provided lock combinations. Contractor control methods shall be designed to preclude unauthorized entry into locked areas and to ensure lock combinations are not revealed to unauthorized person. The Contractor will conduct the training at Malmstrom Air Force Base In Accordance With technical specifications provided on the Statement of Work. Period of Performance will begin on 8 April 2006 and end in September 2008. Classes will be scheduled in accordance to specifications on the Statement of Work. FOB: Destination.
