Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-06-T-0088. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effe ct through Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is 339111. This requirement is 100% total Small Business Set-Aside. The Eisenhower Army Medical Center, Fort Gordon, GA, has a requirement for the following items: CLIN 0001 - 2 each HELMER MODEL PC2200i incubator, platelet, 2-shelf, floor model with four locking casters. i. center integrated monitoring system providing levels of security for valuable platelet products. Adjustable temperature controller is pre-set factory to 22 degrees C for platelet storage. Include a 7 day inkless temperature recorder with a supply of 52 charts. Bacteria resistant powder coated interior and exterior. The unit accomodates 2-PF96i fla tbed agitators. TOTAL PRICE FOR CLIN 0001 $_______ CLIN 0002 - 4 EACH HELMER PF96i agitator, platelet, i.
