Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is issued as a Request For Quotes; RFQ60030. This RFQ is a small business set aside. This solicitation and its incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-09. The National Institute on Drug Abuse (NIDA) Station Support/Simplified Acquisitions (SS/SA) Branch, on behalf of the National Institute of Mental Health intends to negotiate and award a purchase order on competitive bases to rebuild and migrate the existing Laboratory of Brian and Cognition (LBC) Subject Database System to Filmaker V8 standards and further enhance the upgraded system to satisfy the requirements of the system?s plan or action and milestones. The LBC Subject Database is a centralized, networked, Filemaker database system that collects names, contact information, demographics, medical, psychiatric, language, eligibility and availability information for subjects tested under LBC research protocols. The contractor must maintain security and confidentiality identifiers. The data base has information on approximately 915 potential subjects. This information is used as a source pool of available testing subjects and the personally identifiable information collected is used for screening, scheduling and eligibility requirements for LBC study requirements. Subjects are individually screened and information is gathered to determine study eligibility and/or availability. Available ?MRI, MEG, TMS and cognitive testing slots are scheduled and NIH orders are required using the database. The LBC Subject Database Migration Project must adhere to the existing Filemaker screening and scheduling system requirements as specified in LBC Program requirements. The completed LBC Subject Database System must be built at a Filemaker 8 Intermediate level so the LBC IT staff can maintain the system once the project is completed. It is estimated this requirement will need 139 hours and the contractor must be able to complete the project no later than September 15, 2006. In addition, the contractor must be an experienced Filemaker Development Firm (greater than 5 years experience) with the following qualifications: Filemaker 8 Certified, FSA Partner Level Member, and Filemaker 8 Authorized Trainer. The contractor must also be able to maintain security and confidentiality on patient identifier information. The contractor must be able to rebuild and migrate a Filemaker patient scheduling system according to extensive requirements documentation, with weekly solution builds and project updates. The contracted Filemaker Development Firm must also be willing to assist the LBC IT staff throughout the migration/implementation process and be qualified to train the staff in the use of the new database system. Offerors will be evaluated on the following factors: 1) experience in designing and programming sophisticated Filemaker scheduling or project management systems; 2) ability to deliver services according to technical requirements; 3) ability to meet deadlines; and 4) adequacy of staff in experience with FileMaker Pro 8. The anticipated period of performance will be June 30, 2006 to September 15 2006. In order to be considered for an award, contractor must have completed the online electronic Representations and Certifications located at htt;://orca.bpn.gov/ in accordance with FAR 4.1201 (a). By submission of an offer, the contractor acknowledges the requirement that a prospective awardees shall be registered in the CCR at www.ccr.gov/ prior to award, during performance and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. The following Federal Acquisition Regulations (FAR) provisions and clauses apply to this acquisition: FAR 52.212-1 Instructions to Offerors - Commercial Items; FAR 52.212-2 Evaluation - Commercial Item; FAR 52.212-3 Offerors Representations and Certifications - Commercial Items; FAR 52.212-4 Contractor Terms and Conditions - Commercial Items; and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes of Executive Orders ? Commercial Items. The Government intends to select the offer that represents the best value to the Government, price and all other factors considered.
