- This solicitation, VA-255-06-RQ-0021, is issued as a Request for Quotation (RFQ). The associated NAICS code is 621111. The Department of Veterans Affairs, Kansas City VAMC has a requirement with specific requirements to provide ophthalmology surgical procedures, per the Statement of Work, for veterans referred by the Department of Veterans Affairs Medical Center (hereinafter called VAMC), 4801 Linwood Blvd., Kansas City, Missouri 64128. The Solicitation anticipated release date is June 19, 2006. Additional information may be obtained by email from glenn.carpenter@va.gov . Offerors shall submit quotes based on scope of work given in the Solicitation. Contract period is anticipated to begin on August 1, 2006 for a period of one (1) year with the option to renew for (2) additional one year periods. The surgical procedures listed are for services performed at the contractor's location. The Government anticipates award of a Firm Fixed Labor Hour Contract. The offerors should submit all evaluation criteria as documented within the solicitation. In addition the Contractor shall provide their Duns and Bradstreet (DUNS) number and proof of active registration on the Central Contractor Registration (CCR), which is at the website www.ccr.gov. All interested, responsible contractors should submit quotes no later than 4:00 P.M.
