Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The 115 Fighter Wing is soliciting quotes for a Linen Service, to be delivered to the Wisconsin Air National Guard, Truax Field, Madison WI 53704. This requirement is a 100% SMALL BUSINE SS set-aside acquisition. Issued as a Request for Quotation, proposals are being requested and a written solicitation will not be issued. The NAICS code is 812331 and the Standard Industrial Classification (SIC) 7213 applies to the procurement. The cont ractor shall provide the following items OR EQUAL; Bldg. 402 Laundry Bags, 312 each, Bag Stand, 52 each, Bed Sheets, 3037 each, Pillow Cases, 2080 each, Dish Towel, 500 each, Bath Towel, 1664 each; Bldg. 1000, Laundry Bags, 52 each, Shop Towels, 8320 eac h, Coveralls Full Sleeve, 140 each, Coveralls Short Sleeve, 140 each, Bag Stand, 52 each; Bldg. 404, Laundry Bags 52 each, Bag Stand, 52 each, Bath Towel, 2496 each; Bldg. 500, Laundry Bags, 52 each, Bag Stand, 52 each, Apron, 360 each, Bar Towel, 960 eac h, Chef Hats, 120 each, Chef Coats, 120 each. Interested offerors shall contact the Contracting Officer for a copy of the bid schedule. Items are to be delivered FOB Destination to 115 FW, 3110 Mitchell Street, Buildings 402, 404, 500 and 1000, Madison WI 53704. Deliveries will be made to these locations as required. The following provisions are included in this acquisition: FAR 52.212-1, Instruction to Offerors-Commercial Items, Commercial (Jan 2004). The Government will make an award to the respon sible offeror whose offer conforming to the solicitation is the lowest price. Payment will be made via the Government Purchase Card (GPC) a visa card, and will be billed monthly to the individual accounts. Offerors shall include with their quotes a compl eted signed copy of 52.212-3, Offeror Representations and Certifications-Commercial Items (Jun 2006).
