- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA-259-06-RQ-0027 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-11. The North American Industry Classification System (NAICS) number is 339112 - Surgical and Medical Instrument Manufacturing which has a small business size of 500 employees. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. The VA intents to purchase one complete Argon Gas Coagulator for use by the Denver VA GI Lab. This apparatus includes a high-frequency monopolar electrosurgical generator, gas flow meter, flexible delivery catheters, all connector hoses and adapters, ignition tester, a full 40 cu ft argon gas tank (99.998% pure), grounding pad, foot switch(s) and/or remote control to activate both gas and energy, and an initial supply of both parallel and perpendicular directing probes along with other necessary consumables. FOB destination is the Denver VA Medical Center. Forms and clauses stipulated herein may be downloaded via the internet at the following address: http://www.arnet.gov /far/.
